There are few reports of pseudoaneurysm of the facial artery in the literature and very little attention has been paid to their surgical management. Practitioners should be aware of this unusual complication of facial trauma and of the alternatives for treatment of pseudoaneurysms. Surgical resection of selective head and neck pseudoaneurysms provides a safe and effective method for the treatment of these vascular lesions.

Purpose: To evaluate the utility of autogenous extended mandibular ramus and coronoid process bone grafts for maxillofacial reconstructive surgery.

Patients And Methods: Twelve patients aged 23 to 76 years (mean, 52) who underwent extended ramus/coronoid process grafts for reconstruction of maxillofacial deformities due to trauma, alveolar atrophy, or iatrogenic nasal deformity. All patients had either unilateral or bilateral combined coronoid process-mandibular ramus bone grafts for their reconstruction.

Purpose: Cirrhotic patients waiting for liver transplantation who need dental extractions are given fresh frozen plasma and/or platelets to correct coagulopathy. This is costly and may be associated with transfusion reactions and fluid overload. We evaluated the efficacy of intranasal desmopressin as an alternative to transfusion to correct the coagulopathy of cirrhotic patients undergoing dental extraction.

Management of condylar fractures.

Atlas Oral Maxillofac Surg Clin North Am

March 2009

Management of condylar fractures remains a source of ongoing controversy. While it appears that many condylar fractures can be managed nonsurgically, recognition of cases that require surgical intervention and selection of an appropriate procedure are paramount to success in treating these injuries. There are a variety of special considerations that are peculiar to the condylar region.

Purpose: To determine whether any abnormality in serum bone markers is related to bisphosphonate-induced osteonecrosis of the jaw.

Materials And Methods: We obtained serum bone markers and other relevant endocrine assays on 7 patients with osteonecrosis of the jaws (ONJ). The assays were C-telopeptide, N-telopeptide, bone specific alkaline phosphatase, osteocalcin, intact parathyroid hormone, T3, T4, TSH, and vitamin D 25 hydroxy.

We obtained serum bone markers and other relevant endocrine assays on 5 patients with osteonecrosis of the jaw (ONJ). The assays were C-telopeptide, N-telopeptide, bone-specific alkaline phosphatase, osteocalcin, intact parathyroid hormone, T3, T4, TSH, and Vitamin D 25 hydroxy. Diagnostic criteria for ONJ were those formulated by the American Association of Oral and Maxillofacial Surgeons.

Problem Addressed: Bisphosphonates, pyrophosphate analogs, are strong osteoclast inhibitors that are used for osteoporosis and in the treatment of solid tumors with bony metastasis. There are growing reports of osteonecrosis associated with the nitrogen containing bisphosphonates, Pamindronate and Zolendronic acid therapy. Theses cases have been most commonly seen and treated by oral surgeons.

Human gingival keratinocytes in culture stop proliferating after a limited number of passages. This limitation is associated with a gradual depletion of the stem cell compartment of the cell population. Human skin keratinocytes have a three- to five-fold higher proliferation capacity under similar culture conditions, and previous studies indicated that stable down-regulation of the 14-3-3 sigma protein in these cultures prevents stem cell differentiation and generates immortal cell lines without the effects of tumorigenic transformation, e.

A 66-year-old man with multiple myeloma treated with zoledronic acid, melphalan, and prednisone after a tooth extraction developed severe osteonecrosis of the mandible that was nonresponsive to antibiotic therapy. A CT scan showed innumerable fragmented sequestra in the mandible and some adjacent soft tissue fullness around the mandible. The association between the use of bisphosphonates and osteonecrosis of the jaw has been described only recently.

Purpose: The purpose of the study was to evaluate the relationship between mandible fractures and loss of consciousness (LOC) in patients sustaining maxillofacial trauma in a level I trauma center.

Patients And Methods: The study was a retrospective chart review performed at Lincoln Medical and Mental Health Center, a level I trauma center. A total of 225 hospital charts of patients who sustained mandible fractures during the period from 1997 to 2001 were reviewed.

Tinnitus very often is caused by instability of the craniocervical junction. It very frequently manifests as a high-pitched whistle that disappears after operative correction and stabilization of the articular geometry. Prolapsed intervertebral disks, discoligamentous injury, and even metastases as low as level C3 can cause tinnitus, which also usually disappears after surgery.

Lichen planus is a dermatologic disease that affects both skin and mucosa. Here we report five cases of lichen planus that presented as the oral component of the vulvovaginal-gingival syndrome. Four of the cases were associated with biopsy-proven oral lichen planus, and all five patients had oral lesions that clinically resembled lichen planus.

We evaluated objective diagnostic methods for patients with possible upper cervical spine instability caused by trauma and correlated them with subsequent neurosurgical findings and outcomes. Between November 1995 and May 1998, we investigated 420 patients with functional magnetic resonance imaging (MRI) of the craniocervical junction. We evaluated the extracranial vertebral circulation by MRI angiography, with focus on the position of the dens and on the subarachnoid space during entire rotational maneuvers.

Purpose: The goal of this study was to quantify the amount of bone graft material present in the mandibular symphysis as well as to determine the maximal size of the corticocancellous bone block that can be harvested while avoiding mental nerve injury, tooth injury, and simultaneously preserving the preoperative facial contour.

Materials And Methods: Sixteen fixed dentate cadaver mandibles were studied. Osteotomies were performed in a monocortical fashion, 5 mm anterior to the mental foramen, cephalad to the inferior border of the mandible, caudal to the expected position of the apices of the anterior teeth, and at the midline.

Antibiotic prophylaxis for the prevention of infective endocarditis related to dental therapy is a widespread therapeutic intervention. Recently, updated and revised guidelines on this treatment modality have been set forth in the form of consensus statements. It is evident that the risk of spontaneous bacteremia is much greater in a diseased oral cavity than it is in a healthy oral cavity.

The glandular odontogenic cyst is a rare jawbone cyst of odontogenic origin, first described in 1988 by Gardner et al. We describe a previously unreported case of a combined cyst composed of glandular odontogenic cyst and ghost cell keratinization that was present for 33 years. We also reviewed the published 38 cases of glandular odontogenic cyst found in the world literature and add this additional case.
