Background: Delays in intravenous thrombolysis (IVT) treatment for acute ischemic stroke decrease the benefit of treatment. Difficulties determining a patient's clinical eligibility for IVT is a frequent cause of treatment delays.

Objective: We aimed to assess the effectiveness of the "PROVIDENCE" datasheet, a pre- hospital assessment of contraindications for IVT use applied by emergency medical services personnel.

Autoimmune encephalitis is an infrequent pathological occurrence documented within our local context. When clinical suspicion arises, employing electroencephalogram and brain magnetic resonance imaging (MRI) proves valuable. However, for conclusive diagnosis confirmation, lumbar puncture for cerebrospinal fluid (CSF) analysis is indispensable.

Bilateral medial medullary stroke is a very rare condition. It is usually associated with severe motor deficits, dysarthria, sensory symptoms, nystagmus, and respiratory disturbances. The most common etiology is atherosclerotic disease of the vertebral and anterior spinal artery or its branches.

Article Synopsis
  • The review examines the effectiveness of repetitive transcranial magnetic stimulation (rTMS) as a treatment for chronic daily headaches (CDH), highlighting challenges with standard medication.
  • A meta-analysis of 13 clinical trials involving 538 patients found that rTMS significantly reduced headache intensity and frequency immediately after treatment, but the benefits were only marginally sustained in follow-up periods.
  • The study raises questions about the cost-effectiveness of using rTMS regularly for CDH prophylaxis due to variability in treatment protocols and limited long-term efficacy.

Article Synopsis
  • A case study details the successful use of Penumbra's Indigo Aspiration System for endovascular thrombectomy in a patient with cerebral venous sinus thrombosis and hemorrhagic infarct.
  • The patient, a 30-year-old woman, presented with seizures and neurological deficits due to a significant blockage in her venous system, prompting the use of mechanical thrombectomy alongside anticoagulation therapy.
  • The intervention led to a successful recanalization of the affected sinuses and an excellent recovery outcome, highlighting the effectiveness of this minimally invasive device in treating complicated venous thrombosis.

Background: Multiple studies have attempted to determine predictors of poor clinical outcomes in cerebral venous thrombosis (CVT). Fewer studies target to identify predictors of poor response to anticoagulation therapy in CVT.

Objective: We aimed to determine the predictors of poor clinical response to therapeutic anticoagulation in patients with acute CVT.

Introduction: Stroke remains a primary source of functional disability and inpatient mortality in the United States (US). Recent evidence reveals declining mortality associated with stroke hospitalizations in the US. However, data updating trends in inpatient mortality is lacking.

Background: Sexual dysfunction affects at least one half of patients after a stroke. Problems related to sexual function are rarely assessed or addressed by physicians.

Objective: To determine the frequency and factors associated with sexual dysfunction among stroke survivors and self-reported barriers of physicians to discuss sexual dysfunction during regular consultation.

Self-inflicted transorbital stab injuries are not commonly seen in clinical practice. These lesions usually lead to major disability depending on the intracranial structures transected. We present a case of a 34-year-old man with a self-inflicted stab injury in his right orbit from a pen.

Background: Intravenous thrombolysis (IVT) with alteplase is effective in acute ischemic stroke (AIS). However, its use rate remains low due to the many exclusion criteria. Recent guidelines recommend excluding patients suffering AIS with an elevated aPTT secondary to heparin exposure from receiving IVT.

Background: Increasing research reports neurological manifestations of COVID-19 patients. SARS-CoV-2 shares homology with other human coronaviruses that have also had nervous system involvement.

Objective: To review the neurological aspects of SARS-cov2 and other coronavirus, including transmission pathways, mechanisms of invasion into the nervous system, and mechanisms of neurological disease.

Intracranial artery dissection (IAD) is an underdiagnosed, non-atherosclerotic cause of stroke with various clinical manifestations. To identify all the potential studies investigating the epidemiology, risk factors, symptoms, radiology findings, and treatment methods of IAD, we conducted a literature search screening PubMed, SCOPUS, EMBASE, and BIREME. According to the results of several studies, IAD is the major cause of ischemic stroke in at least one-third of the cervical-cranial artery dissection (CCAD) cases presenting with ischemic stroke.
