Article Synopsis
  • Over the past 20 years, there's been a rise in new treatments for inflammatory bowel disease (IBD), significantly increasing therapy costs, which raises concerns among healthcare stakeholders.
  • The study involved 53 IBD patients in Bucharest who were switched from the original biologic treatment to a biosimilar of Adalimumab to evaluate clinical outcomes and side effects after one year.
  • Results indicated no significant changes in disease markers like fecal calprotectin and C-reactive protein after the switch, suggesting that the biosimilar is as effective and safe as the original Adalimumab, with minimal side effects reported.

Background/aims: Gastrointestinal bleeding is a significant and potentially lethal event. We aimed to review the efficiency and safety of self-assembling peptides for the treatment and prevention of gastrointestinal tract bleeding.

Methods: We conducted a systematic search for studies describing the endoscopic use of self-assembling peptides for treatment or prevention of bleeding in the gastrointestinal tract in a parallel, independent fashion.

Background: Hepatitis B, a vaccine-preventable liver infection, remains a global public health problem. Dedicated groups of experts and funding are focusing on achieving a functional cure to eradicate this disease by 2030.

Areas Of Uncertainty: With more than 40 molecules available or under investigation as new treatments for hepatitis B virus (HBV) infection, none of them is curative so far.

Article Synopsis
  • A study was conducted in Romania to evaluate the safety, efficacy, and treatment persistence of five advanced therapies for inflammatory bowel disease (IBD) in a retrospective analysis involving 678 patients from 24 centers.
  • Most patients were treated with vedolizumab, while a significant number had prior treatment failures.
  • Results indicated that 71% of patients achieved clinical remission and 36% achieved mucosal healing, with 78% persistence at 6 months and no fatal adverse reactions recorded.

Background: Histologic activity has emerged as an aspirational therapeutic goal in ulcerative colitis management. It is not yet a formal treatment target in ulcerative colitis. However, it could be used as an adjunct to mucosal healing to represent a deeper level of healing.

Background And Aims: MicroRNAs (miR) have altered expression in multiple autoimmune disorders including inflammatory bowel disease. The aim of the study was to assess the tissue and circulating miR-31, miR-200b, and miR-200c expression levels as potential biomarkers for intestinal disease activity in patients with Crohn's disease (CD).

Methods: The study included 45 patients with histopathological confirmed CD and active disease (defined as fecal calprotectin >50 μg/g and Simple Endoscopic Score (SES) of CD >3), and 21 subjects as controls for the validation cohort.

Crohn's disease and ulcerative colitis remain debilitating disorders, characterized by progressive bowel damage and possible lethal complications. The growing number of applications for artificial intelligence in gastrointestinal endoscopy has already shown great potential, especially in the field of neoplastic and pre-neoplastic lesion detection and characterization, and is currently under evaluation in the field of inflammatory bowel disease management. The application of artificial intelligence in inflammatory bowel diseases can range from genomic dataset analysis and risk prediction model construction to the disease grading severity and assessment of the response to treatment using machine learning.

Article Synopsis
  • The growth of advanced therapies for inflammatory bowel disease has led to a disconnect between their development and practical use due to unclear definitions for treatment success, failure, and when to stop therapy.
  • A systematic review identified that criteria for treatment response vary widely among studies, with inconsistent definitions for remission and differing assessment timelines across trials.
  • The absence of standardized guidelines results in reliance on empirical strategies, which can lead to premature cessation of effective therapies for patients.

Background: Mucosal healing (MH) has emerged as a key therapeutic target in inflammatory bowel disease (IBD), and achievement of this goal is documented by endoscopy with biopsy. However, colonoscopy is burdensome and invasive, and substitution with an accurate noninvasive biomarker is desirable.

Aim: To summarize published data regarding the performance of noninvasive biomarkers in assessing MH in IBD patients.

Article Synopsis
  • Adequate sedation is essential for quality digestive endoscopy, and this study assessed sedation practices and safety in Romania, focusing on adverse events and procedure success rates.
  • A total of 1,043 procedures were analyzed, revealing that over half were sedated by an anaesthesiologist, with a low rate of sedation-related adverse events (3.8%) mostly being mild and self-resolving.
  • The study highlighted that establishing a national guideline for sedation could standardize practices and enhance safety across different endoscopy units in Romania.

Biliopancreatic tumors (BPT) are among the most aggressive solid malignancies, and their incidence is rising. Good patient outcome relies heavily on a multidisciplinary approach to therapy, including timely access to endoscopy, surgery and chemo/radiotherapy. We aimed to evaluate current practices as reflected in the management and outcome of patients diagnosed with BPT in the setting of a low-resource medical system in order to identify areas suitable for improvement.

Background: To date, no scale has been validated to assess bubbles associated with bowel preparation. This study aimed to develop and assess the reliability of a novel scale - the Colon Endoscopic Bubble Scale (CEBuS).

Methods: This was a multicenter, prospective, observational study with two online evaluation phases of 45 randomly distributed still colonoscopy images (15 per scale grade).

Objective: Our aim was to prospectively assess bowel preparation in patients with inflammatory bowel disease (IBD) and to determine the impact of disease-related factors on preparation efficacy because few studies have addressed this issue.

Methods: We conducted a retrospective analysis of data collected from a cohort of patients with IBD enrolled at a tertiary center in Bucharest, Romania. Patients were evaluated every 12 months, with each study visit including collection of clinical, biological, and endoscopic data.

Endoscopy has a central role in the management of inflammatory bowel disease (IBD), providing crucial data for diagnostic and therapeutic decisions, treating disease-related complications, and assisting in the early detection of dysplasia and prevention of colorectal cancer in the setting of IBD. Treatment targets have significantly shifted in IBD, focusing on achieving mucosal healing, a more meaningful endpoint than clinical remission. With the emergence of novel therapies, we aim to alter the course of the disease and prevent irreversible damage to the bowel.
