Extracorporeal circulation (ECC) support using intraoperative extracorporeal membrane oxygenation (ECMO) during lung transplantation (LTx) is now a routine practice for many high volume centers. Circuits that are dedicated to ECMO alone can be expensive and do not allow full cardiopulmonary bypass (CPB) to be performed. We describe our technique of instituting venoarterial ECMO during LTx using a less-expensive hybrid circuit that facilitates easy and immediate conversion to full CPB if needed, without interruption of ECC.

Purpose Of Review: The aim of this article is to review current practice of spinal anesthesia regarding technique and medication use; review recent applications of spinal anesthesia to subspecialty care in outpatient, cardiac, and obstetrical anesthesia; and update risk assessment associated with spinal anesthesia.

Recent Findings: Epidural volume extension enhances the spread of local anesthetics using a combined spinal-epidural technique. Chloroprocaine has become the agent of choice at some institutions.

This article discusses the current state of minimally invasive treatment options for a variety of vascular diseases. Advances that have been introduced over the last two decades have dramatically changed the practice of vascular surgery and anesthesia. The ability to treat pathology, using both intraluminal and extraluminal means,has provided vascular surgeons, interventional radiologists, and cardiologists with unique treatment options that were not available less than a decade ago.
