Mitochondrial diseases are a clinically hetyerogenous group of disorders. They can be caused by mutations of nuclear or mitochondrial DNA (mtDNA). Some affect a single organ, but many involve multiple organ systems and often present with prominent neurologic and myopathic features.

A 35-year-old Kuwaiti lady, who is a known myope, developed severe anterior segment ischemia following extra-ocular muscle transposition surgery. The patient was treated with topical and systemic steroids. Her best corrected visual acuity after complete resolution of the inflammation was 20/40.
