Epididymo-Orchitis Mimicking Malignancy Resulting From Intravesical Bacillus Calmette-Guerin Immunotherapy for Bladder Cancer: An Attempt to Understand Pathophysiology, Diagnostic Challenges, Patients' Implications and Future Directions.

This case report describes an 85-year-old patient who presented with painless, unilateral right testicular swelling of 2 months' duration. This raised the possibility of testicular cancer, especially given his recent treatment for bladder cancer, which included adjuvant intravesical bacillus Calmette-Guerin (BCG) therapy. This poses a diagnostic dilemma regarding tuberculosis (TB) of the testis, BCG complications or a true testicular malignancy.

A greenhouse pot experiment was conducted with seven different levels of sludge (0, 5, 10, 20, 40, 80, 160 g kg) to assess the potential impact of sludge application on soybean (Glycine max (L.) Merr.) productivity, metal accumulation and translocation, and physico-chemical changes in acid and alkaline soils.

We report a case of a 46-year-old female with HIV for the last 15 years admitted to our hospital with a history of fever. She was diagnosed with pneumonia and was treated with antibiotics and recovered well; however, she was found to be hyponatremic. The SARS-CoV-2 Omicron variant has challenged demands to minimise workplace transmission in healthcare settings while maintaining adequate staffing. Policymakers have shortened COVID-19 isolation periods, although little real-world data have evaluated the utility. Our findings from surveillance of 240 healthcare workers from Sheffield Teaching Hospitals, England, show that 55% of affected staff could return before day 10 of isolation with over 25% eligible on day 6, pending two successive negative antigen tests.

Randomized Controlled Trials to Define Viral Load Thresholds for Cytomegalovirus Pre-Emptive Therapy.

Background: To help decide when to start and when to stop pre-emptive therapy for cytomegalovirus infection, we conducted two open-label randomized controlled trials in renal, liver and bone marrow transplant recipients in a single centre where pre-emptive therapy is indicated if viraemia exceeds 3000 genomes/ml (2520 IU/ml) of whole blood.

Methods: Patients with two consecutive viraemia episodes each below 3000 genomes/ml were randomized to continue monitoring or to immediate treatment (Part A). A separate group of patients with viral load greater than 3000 genomes/ml was randomized to stop pre-emptive therapy when two consecutive levels less than 200 genomes/ml (168 IU/ml) or less than 3000 genomes/ml were obtained (Part B).

Bilateral paratesticular liposarcoma - a rare case report.

Paratesticular liposarcomas are rare tumors and are often reported as isolated cases. Patients usually present with a painless scrotal or inguinal mass, mimicking inguinal hernia. They refer to liposarcomas arising from the spermatic cord, testicular tunics, and epididymis.

Adenocarcinoma in an ileal duplication.

Alimentary tract duplications are a rare congenital anomalies that usually present in childhood and occasionally in adults. To our knowledge, since 1884, there have been only 14 reported cases of ileal duplications in adults of which 3 patients had developed malignancy in the duplicated ileum. We report here such an extremely rare case of isolated ileal duplication in an elderly male in whom malignancy occurred and perforated.
