Nonalcoholic fatty liver disease (NAFLD) is the most common chronic hepatic disease worldwide, with functional impairment of the mitochondria occurring from early stages. Technetium-99m methoxy-isobutyl-isonitrile (Tc-MIBI) is a lipophilic agent trapped in the mitochondria. This study aims to evaluate the utility of Tc-MIBI heart/liver uptake ratio in screening for NAFLD during myocardial perfusion imaging (MPI).

Chylous reflux is described as the backflow of chyle through the varicose lymphatics with incompetent lymph valves and leakage of the lymph. The valves in the dilated, varicose, tortuous lymph vessels become incompetent, and as a result, the lymph blended with chyle, reflux into the lower limbs or the genitalia. Reflux causes delayed lymphatic transport in the affected lower extremity and chronic lymphedema can develop.

We report a case of spontaneous biliary perforation in a 21-month old female pediatric patient with the presenting symptoms of abdominal distension, jaundice, and umbilical hernia. Hepatobiliary scintigraphy showed tracer accumulation in the peritoneal cavity. Preliminary diagnosis of spontaneous biliary perforation was confirmed during surgery.
