Article Synopsis
  • Emergency endoscopic hemostasis is effective for treating colonic diverticular bleeding, but identifying the bleeding source can be challenging for patients and healthcare providers.
  • A scoring system called the Jichi Medical University diverticular hemorrhage score (JD score) was developed to better predict bleeding sources based on specific clinical criteria.
  • Results showed that a higher JD score significantly increased the rate of identifying the bleeding source (77% vs. 23%), ultimately leading to shorter hospital stays, but there was a greater incidence of rebleeding in high JD score patients.

Most cases of liver dysfunction in pregnancy are pregnancy-related, but the onset of systemic autoimmune diseases is also differentiated. A 24-year-old woman presented with liver dysfunction at 28 weeks' gestation with suspected autoimmune hepatitis and started taking ursodeoxycholic acid. She gave birth prematurely at 35 weeks' gestation, and the infant presented with pancytopenia and liver failure but survived because of liver transplantation.

Aim: Subepithelial lesions (SELs) are defined as being located under the mucosa. Presently, endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) is commonly performed to diagnose SELs. With the development of new puncture needles, endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB), which allows for the acquisition of large tissue samples, has been proposed.

Objective: The objective of this study was to clarify the characteristics and management of painless biliary type sphincter of Oddi dysfunction (SOD).

Methods: From June 2002 to July 2018, 12 patients who had recurrent liver dysfunction with a dilated bile duct or acute cholestasis of unknown cause without biliary pain (painless SOD) were included in this study. These patients’ characteristics were compared with those of 36 patients with biliary type SOD based on the conventional definition (criteria-based SOD).

Background And Objectives: Tacrolimus is very effective at inducing remission in patients with refractory ulcerative colitis. However, the optimal time-point for the discontinuation of tacrolimus is controversial because administration of tacrolimus for > 3 months is currently not approved for insurance reimbursement in Japan. We conducted this study to determine the optimal time of discontinuation of tacrolimus in patients with ulcerative colitis.

The patient was a "73" -year-old woman who visited our hospital with the chief complaint of weight loss. Upper gastrointestinal endoscopy revealed an enlarged ampulla of Vater, and a biopsy led to a diagnosis of Group "4" gastric carcinoma; suspicious of adenocarcinoma. There were no findings suggesting invasion into the muscle layer of duodenum, despite tumor mass formation being observed in the sphincter of Oddi.

Article Synopsis
  • Gastrointestinal hormones play a crucial role in maintaining intestinal homeostasis and regulating processes like digestion, absorption, and excretion, primarily acting through G-protein-coupled receptors.
  • Recent research indicated that lacking Gα/Gα signaling negatively impacts the maturation of Paneth cells, leading to their differentiation into goblet cells and affecting colonic mucosa regeneration in colitis models.
  • In experiments with an intestinal epithelial cell line, manipulating Gα/Gα levels showed that overexpression inhibited cell growth, while downregulation promoted it, hinting at a potential role of Gα/Gα signaling in regulating enterocyte functions like absorption or secretion.
