Purpose: To evaluate the feasibility and efficacy of a modified metallic coil embolization technique for pulmonary arteriovenous malformations (PAVMs) using coil anchors and occlusion balloon catheters.

Methods: Five consecutive female patients with 6 PAVMs were included in the present study. One of the 6 PAVMs was of the complex type with four feeding arteries.

Preoperative evaluation of the right gastroepiploic artery was performed by abdominal ultrasonography to determine whether the graft was adequate for coronary artery bypass. The gastroepiploic artery was used when the graft diameter was greater than 2 mm with pulsatile flow. Postoperative angiography revealed the gastroepiploic artery dominant, and the graft diameter measured by angiography was almost the same size as that of abdominal ultrasonography.

A 64-year-old woman with a history of cerebral infarction from chronic occlusion of the middle cerebral artery presented with acute myocardial infarction. Double, off-pump coronary artery bypass grafting could not prevent stroke caused by atheroembolism from the ascending aorta. The risk of stroke from atheroembolism should be taken into consideration when planning coronary artery bypass grafting.
