Objective: Contrast-induced encephalopathy (CIE) is a rare but severe complication that can occur following intravascular treatment of intracranial vascular disease. Although CIE is considered a transient neurological disorder, its natural history, pathophysiology, and risk factors are poorly understood. Contrast leakage (CL) is a more frequently observed adverse event than CIE and can lead to CIE.

Article Synopsis
  • Endovascular treatment is common for subclavian artery issues, but precautions are necessary to prevent complications, especially concerning the vertebral artery.
  • A case involving a 78-year-old woman revealed complications when a protection device got trapped in a stent due to patient movement during the procedure.
  • Successful retrieval required open surgery, highlighting the need for careful technique and consideration of anesthesia to avoid such issues in future procedures.

Objective: Antithrombotic medications pose a challenge for conducting surgical or invasive procedures, because their discontinuation is required to avoid postprocedural hemorrhagic complications but potentially increases the ischemic risk for the patient. This study aimed to estimate the increased risk of developing cerebral ischemic events during hospitalization requiring discontinuation of antithrombotic therapy.

Methods: This investigation was a single-center retrospective observational study.

The development of clinical practice guidelines for social anxiety disorder began in March 2018 when the Joint Clinical Practice Guideline Development Committee for Anxiety and Obsessive-Compulsive Disorders was formed by the Japanese Society of Anxiety and Related Disorders and Japanese Society of Neuropsychopharmacology to jointly develop guidelines for anxiety and obsessive-compulsive disorders. Based on the universal concept of evidence-based medicine, three clinical questions (CQs) about pharmacotherapy and psychotherapy were developed for clinical guidelines for social anxiety disorder, panic disorders, and obsessive-compulsive disorder in accordance with the Minds "Manual for Guidelines Development 2017 by the Japan Council for Quality Health Care: CQ1-"What is the recommended pharmacotherapy for social anxiety disorder in adults?"; CQ2-"What is the recommended psychotherapy (psychological intervention) for social anxiety disorder in adults?"; and CQ3-"What are the recommendations regarding monotherapy and combination therapy for social anxiety disorder in adults in terms of pharmacotherapy and psychotherapy (psychological interventions)?" Summarized recommendations for social anxiety disorder in adults are selective serotonin reuptake inhibitors and serotonin-norepinephrine reuptake inhibitor for CQ1, cognitive behavioral therapy for CQ2, and there are no recommendations regarding monotherapy and combination therapy for CQ3. These were answered by considering the balance between benefits and harms based on systematic reviews of each.

Article Synopsis
  • - The study investigates how mechanical thrombectomy (MT) outcomes differ when measured by the modified Rankin Scale (mRS) versus the Functional Independence Measure (FIM), highlighting the potential limitations of mRS in assessing patient independence.
  • - Results showed that while 33% of patients had mRS scores indicating good recovery, only 15% achieved independence according to FIM, suggesting that the FIM may show a more realistic measure of patient functionality post-MT.
  • - Significant factors affecting mRS outcomes included hospitalization duration and NIHSS scores, while only the NIHSS score was notably linked to achieving a high FIM score, indicating distinct clinical backgrounds influence these assessment tools differently.

Systemic amyloidosis is a diseased condition where misfolded proteins deposit in various organs in the form of amyloids, and transthyretin deposition, termed ATTR amyloidosis, can be either an age-related amyloid formation from misfolded wild-type TTR (ATTRwt) or by hereditary TTR malfunction due to mutation in the TTR gene (ATTRv). Although ATTRwt amyloidosis can cause various diseases, such as cardiac failure, conduction disturbances, arrhythmias and carpal tunnel syndrome, it is still under-recognised considering its clinical significance. Here the authors report a case of ATTRwt amyloidosis leading to carotid stenosis requiring surgical intervention.

The current study aimed to test whether the ratio of T1-weighted to T2-weighted signal intensity (T1W/T2W ratio: rT1/T2) derived from conventional MRI could act as a surrogate relaxation time predictive of IDH mutation status in histologically lower-grade gliomas. Strong exponential correlations were found between rT1/T2 and each of T1- and T2-relaxation times in eight subjects (rT1/T2 = 1.63exp + 0.

Article Synopsis
  • Eagle's syndrome can lead to internal carotid artery dissection, but treatment options are not clearly defined.
  • A 72-year-old man with symptoms like temporary speech issues and impaired consciousness was found to have multiple strokes linked to this syndrome and underwent carotid artery stenting (CAS) after a blood flow assessment.
  • The successful treatment with CAS improved his blood flow and neurological condition, suggesting it may be a viable option for similar cases caused by hemodynamic stress.

Minitablets have garnered interest as a new paediatric formulation that is easier to swallow than liquid formulations. In Japan, besides the latter, fine granules are frequently used for children. We examined the swallowability of multiple drug-free minitablets and compared it with that of fine granules and liquid formulations in 40 children of two age groups ( = 20 each, aged 6-11 and 12-23 months).

Most atypical teratoid/rhabdoid tumor (AT/RT) of the central nervous system shows an inactivation of SMARCB1 (INI1) and is considered as the hallmark of this neoplasm. However, AT/RT could exceptionally rarely present retained SMARCB1 (INI1) but inactivated SMARCA4 (BRG1). Here, the authors report a rare case of a 2-year-old boy with a SMARCB1 (INI1) retained but SMARCA4 (BRG1) negative AT/RT arising at the bilateral cerebellopontine angles mimicking neurofibromatosis type 2.
