Background: Over 95% of penicillin allergy labels are inaccurate and may be addressed in low-risk patients using direct oral penicillin challenge (DPC). This study explored the behaviour, attitudes and acceptability of patients, healthcare professionals (HCPs) and managers of using DPC in low-risk patients.

Methods: Mixed-method, investigation involving patient interviews and staff focus groups at three NHS acute hospitals.

Wiskott-Aldrich syndrome (WAS) is a multifaceted monogenic disorder with a broad disease spectrum and variable disease severity and a variety of treatment options including allogeneic hematopoietic stem cell transplantation (HSCT) and gene therapy (GT). No reliable biomarker exists to predict disease course and outcome for individual patients. A total of 577 patients with a WAS variant from 26 countries and a median follow-up of 8.

Article Synopsis
  • The study highlights the burden of inaccurate penicillin allergy labels (PALs) and explores the potential for non-allergy healthcare professionals to conduct direct oral penicillin challenges (DPCs) to de-label patients.
  • Out of 2257 screened patients, 270 consented to participate, with 126 low-risk patients undergoing DPC and achieving a 96.8% de-labeling rate without serious reactions.
  • The research suggests that DPCs are feasible for non-specialists and emphasizes the need for strategies to improve patient enrollment, particularly in elective medical settings.

Histone deacetylase 6 (HDAC6) contributes to cancer metastasis in several cancers, including triple-negative breast cancer (TNBC)-the most lethal form that lacks effective therapy. Although several efforts have been invested to develop selective HDAC6 inhibitors, none have been approved by the FDA. Toward this goal, existing computational studies used smaller compound libraries and shorter MD simulations.

  • When he switched back to intravenous vedolizumab, he experienced a new hypersensitivity reaction, leading to the complete discontinuation of the medication.
  • This case emphasizes the importance of being cautious when switching back to intravenous vedolizumab after an allergic reaction to the subcutaneous form, even if the patient had no issues with the intravenous version before.

Macrophage activation and hypercytokinemia are notable presentations in certain viral infections leading to severe disease and poor prognosis. Viral infections can cause macrophage polarization into the pro-inflammatory M1 or anti-inflammatory M2 phenotype. Activated M1 macrophages usually restrict viral replication whereas activated M2 macrophages suppress inflammation and promote tissue repair.

Article Synopsis
  • In March 2020, the UK Primary Immunodeficiency Network created a registry to track outcomes of individuals with Primary Immunodeficiency Diseases (PID) and Secondary Immunodeficiency Diseases (SID) after infection with SARS-CoV-2, reporting 310 cases.
  • The overall mortality rate was 17.7%, with certain groups experiencing higher rates; for example, Common Variable Immunodeficiency Disease (CVID) had an infection fatality rate (IFR) of 18.3%.
  • Individuals with PID and SID faced greater risks in terms of inpatient mortality and generally died at younger ages than the broader population, with risk factors including older age, low lymphocyte count before infection, and existing co-morbidities

Aedes albopictus is one of the main mosquito vectors responsible for transmitting arboviruses to humans and animals. The ability of this mosquito to support virus transmission has been linked to vector competence, which is partly attributed to the genetic disparities in Ae. albopictus population.

Case Presentation: A 68-year-old woman was referred for immunological investigation following an episode of meningococcal epiglottitis with associated septicaemia. Several years previously, she had been diagnosed with undifferentiated connective tissue disease. On investigation, alternative pathway complement function was normal; however, classical pathway complement activation was reduced.

Tocilizumab (TCZ), an IL-6 receptor antagonist, is used in the treatment of severe COVID-19 caused by infection with SARS-CoV-2. However, unintended consequences of TCZ therapy include reactivation of tuberculosis (TB) or hepatitis B virus (HBV), and worsening of hepatitis C virus (HCV). We set out to assimilate existing data for these complications, in order to help inform evidence-based risk assessments for the use of TCZ, and thus to reduce the risk of serious but preventable complications.

The recently emerged coronavirus disease (COVID-19), which has been characterised as a pandemic by the World Health Organization (WHO), is impacting all parts of human society including agriculture, manufacturing, and tertiary sectors involving all service provision industries. This paper aims to give an overview of potential host reservoirs that could cause pandemic outbreak caused by zoonotic transmission. Amongst all, continues surveillance in slaughterhouse for possible pathogens transmission is needed to prevent next pandemic outbreak.

Despite increasing interest in pharmacogenomics, and the potential benefits to improve patient care, implementation into clinical practice has not been widespread. Recently, there has been a drive to implement genomic medicine into the UK National Health Service (NHS), largely spurred on by the success of the 100,000 Genomes Project. The UK Pharmacogenetics and Stratified Medicine Network, NHS England and Genomics England invited experts from academia, the healthcare sector, industry and patient representatives to come together to discuss the opportunities and challenges of implementing pharmacogenomics into the NHS.

Chloroquine (CQ) and its derivatives such as hydroxychloroquine (HCQ) remain mainstay of therapy for malaria. These drugs are also approved for certain autoimmune diseases including systemic lupus erythematosus. The antiviral activities of these drugs and their mechanisms have been studied in vitro previously against various viruses including severe acute respiratory syndrome coronavirus (SARS-CoV).
