A reactive nodular fibrous pseudotumor (RNFP) is a rare benign tumor that is related to previous abdominal surgery, injury, or inflammation, and in patients with a history of malignancy; it is easily misdiagnosed as tumor recurrence or metastasis. We discuss the case of an 80-year-old patient who presented to the emergency department with a large non-reducible inguinal hernia. The patient was taken to the operating room for an open hernia repair through an inguinal incision.

Article Synopsis
  • A 58-year-old woman experienced abdominal pain, vomiting, and constipation, with lab tests showing elevated white blood cells and C-reactive protein.
  • CT imaging revealed a large cystic mass in the right ovary, leading to a suspicion of ovarian cancer, but her condition worsened despite conservative treatment.
  • Surgery ultimately revealed a perforated dermoid ovarian cyst, which was resected, and a prophylactic ileostomy was performed; histopathology confirmed it was benign, highlighting the need to consider such conditions in similar cases.

Spontaneous splenic rupture (SSR) is a relatively rare but potentially lethal complication of infectious mononucleosis (IM). While SSR is extremely rare in patients with proven IM, it is the most lethal complication of the infection (9% mortality rate) and can present completely asymptomatically or with abdominal pain and hemodynamic instability. As adolescents and young adults are the most affected population group, with this case report, we intend to raise the vigilance of any doctor treating those patients in the emergency department.
