Postoperative atrial fibrillation complicates 15-40% of cardiac surgery cases and is associated with various adverse health outcomes including high mortality. Although vasopressin administration decreases postoperative atrial fibrillation in on-pump coronary artery bypass grafting, its use in off-pump coronary artery bypass grafting has not been investigated. A 90-year-old woman with severe aortic stenosis experienced hospital readmission for chronic heart failure exacerbations many times and was admitted to our hospital for undergoing transcatheter aortic valve implantation. Thereafter, she fell in the ward and fractured her femoral trochanter, requiring early hip fracture surgery. We proposed that we should perform simultaneous transcatheter aortic valve implantation and hip fracture surgery to cardiologist and orthopedist from anesthetic and perioperative management perspective.

We report a case of complete heart block induced by insertion of a pulmonary artery catheter (PAC). A 74-year-old man was scheduled for off-pump coronary artery bypass grafting. The preoperative electrocardiogram revealed complete left bundle-branch block (LBBB).

We report alteration of the planned surgical procedures in two cases during emergency operations. Both patients were urgently scheduled for off-pump coronary artery bypass grafting surgery supported by an intraaortic balloon pump. Intraoperative transesophageal echocardiography (TEE) demonstrated the findings which had not been diagnosed preoperatively and the operative procedure was altered in both cases.
