Introduction: The accurate distinction between benign and malignant biliary strictures (BS) poses a significant challenge. Despite the use of bile duct biopsies and brush cytology via endoscopic retrograde cholangiopancreaticography (ERCP), the results remain suboptimal. Single-operator cholangioscopy can enhance the diagnostic yield in BS, but its limited availability and high costs are substantial barriers.

Article Synopsis
  • The DDOST protein is crucial for the N-linked glycosylation process in the endoplasmic reticulum and is linked to various cancers and congenital glycosylation disorders.
  • Research shows that knocking down DDOST expression in pancreatic cancer cells (specifically PDAC) leads to changes in protein expression and interactions, pointing to its potential role in the disease.
  • Results indicate that DDOST knockdown decreases cancer cell proliferation and viability while increasing stress responses and apoptosis, suggesting that DDOST may act as an oncogene and could be targeted for therapeutic strategies in pancreatic cancer.

Background: A standardised dose-reduction strategy has not been established for the widely used gemcitabine plus nab-paclitaxel regimen in patients with metastatic pancreatic ductal adenocarcinoma. We aimed to investigate the efficacy and tolerability of alternating treatment cycles of nab-paclitaxel-gemcitabine combination therapy and gemcitabine alone versus continuous treatment with the nab-paclitaxel-gemcitabine combination.

Methods: ALPACA was a randomised, open-label, phase 2 trial conducted at 29 study centres across Germany.

Importance: The prognosis of patients with adenocarcinoma of the esophagus and esophagogastric junction (AEG) is poor. From current evidence, it remains unclear to what extent preoperative chemoradiotherapy (CRT) or preoperative and/or perioperative chemotherapy achieve better outcomes than surgery alone.

Objective: To assess the association of preoperative CRT and preoperative and/or perioperative chemotherapy in patients with AEG with overall survival and other outcomes.

Pancreatic ductal adenocarcinoma (PDAC) is difficult to diagnose in the early stages and lacks reliable biomarkers. The scope of this project was to establish quantitative nuclear magnetic resonance (NMR) spectroscopy to comprehensively study blood serum alterations in PDAC patients. Serum samples from 34 PDAC patients obtained before and after pancreatectomy as well as 83 age- and sex-matched control samples from healthy donors were analyzed with diagnostics research (IVDr) proton NMR spectroscopy at 600 MHz.

The acute phase protein Serum Amyloid A (SAA) is synthesised by the liver in response to inflammatory stimuli. Previous studies have revealed that SAA may be a better biomarker of disease activity in inflammatory bowel disease (IBD) compared to C-reactive protein (CRP). This retrospective monocentric study evaluated whether SAA correlates with biomarkers like faecal calprotectin (FC), CRP, the Neutrophil to Lymphocyte ratio (NLR), the platelet count and clinical disease activity of IBD patients.

Article Synopsis
  • The study aimed to determine if observing tissue transition in liver lesion biopsies could predict the success of these procedures, as assessed by modified macroscopic on-site evaluation (MOSE).
  • A retrospective analysis of 264 liver biopsies showed that when tissue transition was visible, the rates of successful material retrieval and conclusive diagnosis were significantly higher.
  • The findings suggest that incorporating MOSE during biopsies could enhance diagnostic accuracy and be particularly helpful in settings without immediate access to a pathologist.
