Lysis of deep venous thrombotic obstruction in the extremities has been shown to decrease the frequency and severity of symptoms of postthrombotic syndrome. However, for those lesions that contain organized thrombus, the use of conventional methods to lyse thrombus is less successful than for acute thrombus. The authors here present a novel method for treatment of older, organized thrombus using excimer laser.

The treatment of superficial venous disease and chronic venous insufficiency continues to evolve, and the interest in venous disease has matched that in arterial disease in vascular medicine. A better understanding of venous anatomy and pathophysiology and the development of newer, more efficient diagnostic technology have allowed clinicians to utilize minimally invasive techniques in the treatment of varicose veins. These techniques have reduced recurrence and improved overall quality of life (postoperative pain and bruising) following these procedures.

Exposed or infected peripheral vascular grafts pose a significant challenge to the vascular surgeon. Although graft removal and extraanatomic bypass is feasible in selected circumstances, this procedure is generally not applicable for bypass to the pedal vessels. Preservation of patent grafts is almost always required for limb salvage.
