In 77 patients with critical limb ishaemia treated surgically (arterialization of venous blood flow, creation of arteriovenous fistulas, transplantation of great omentum on microvascular anastomosis) regional perfusion of both legs was evaluated with radionuclide angiography. High perfusion of the ishaemic limb was revealed in the majority of patients. In these patients positive results were achieved most often, particularly in creation of arteriovenous fistulas on the leg low third and foot.

A 133Xe radionuclide investigation was employed for examination of regional cerebral circulation with 5% carbogen mixture loading in patients with occlusions of the aortic arch major arteries. It was established that a positive response to carbogen mixture inhalation indicates a compensated cerebral circulation irrespective of the arterial lesion type and number. The negative vascular response is associated with uni- or bilateral reduction of the regional cerebral circulation depending on the type and severity of the arterial involvement.

The authors show the significance of radionuclide angiography in evaluating the efficacy of surgical treatment of lower limb critical ischemia by formation of an arteriovenous anastomosis on the side of the affection. An operation for formation of an arteriovenous anastomosis on the lower third of the leg or the foot was found to be most effective. Increased perfusion of the ischemic limb on the level of balanced mixing of the blood flow was revealed in most patients, which the authors believe to be caused by venous stasis in the lower limb due to reduced difference in pressure in the arterial and venous bed.

Radionuclide equilibrium ventriculography was used to examine general and regional ejection fractions and velocity characteristics for both cardiac ventricles in 28 patients with occlusions of the aortic arch branches consequent to aortic arteritis of atherosclerosis. In nonspecific aortic arteritis (NAA) even young patients have myocardial dysfunction comparable to that in the elderly with atherosclerosis. However, as shown by the nitroglycerin test, these patients preserve functional reserves of the myocardium.

The informative value of determination of the restoration rate of spiroergometric parameters was examined in cardiosurgical patients who had undergone exercise testing. All the patients were divided into 3 homogeneous groups: (1) those without disorders; (2) those with moderate restoration; and (3) pronounced restoration. There was a moderate relationship between the restoration rate and the function of the oxygen transport system (OTS) and a high relationship between the rate and exercise tolerance in decompensated metabolic acidosis and left ventricular function.

The goal of the present paper was a comparative study of microcirculation in the peritoneum, mesentery and omentum during the development of experimental peritonitis, as well as the investigation of possible correction of microcirculatory disturbances with the help of hemosorption. It has been established that different degree of microcirculatory disturbances in the tissues under study depended on the duration of peritonitis. Hemosorption recovered microcirculation to initial values, which indicated the reversibility of the disturbances.

An analysis of results of spiroergometry and radionuclide ventriculography in the mixed group of cardiosurgical patients allowed to reveal a reliable accretion of fraction of expulsion of the left ventricle during transition from the state of rest to work at the level of anaerobic threshold and absence of accretion of this index in further increase of exercise. The results obtained allowed to propose the use of anaerobic threshold equal to 60% of the threshold uptake of oxygen as an indicator in choosing the optimum intensity of the training exercise of patients operated on for heart diseases.

Under analysis are results of operative treatment of 54 patients with cirrhosis of the liver. Most satisfactory hemodynamic results were obtained after endovascular embolisation of the splenic artery as the increased general hepatic blood flow and simultaneously decreased splenic blood flow, better indices of intracardiac hemodynamics. The method of peritoneoatrial shunting is most perspective in patients with cirrhosis of the liver at the stage of decompensation of portal hypertension.

Isotope angiography was fulfilled in all the 38 patients, ultrasonography was used in 20 patients. The analysis of hemodynamic effect of closed mitral commissurotomy in 73 patients it different postoperative terms is presented. Within the first day after operation a dramatic hyperfunction of the myocardium was observed in patients with the 3d stage of blood circulation disorders. In patients with the 4th stage of the disease mitral commissurotomy failed to give a positive hemodynamic effect both within the first day and at later terms.
