Background Endovascular thrombectomy (EVT) is an effective method for vertebrobasilar artery occlusion (VBAO) in patients with moderate to severe deficits but has unclear benefits in patients with low National Institutes of Health Stroke Scale (NIHSS) scores at hospital admission. Purpose To compare the clinical outcomes of best medical management (BMM) alone versus BMM and early EVT (door-to-puncture time [DPT] ≤ 120 minutes) versus BMM and late EVT (DPT > 120 minutes) in patients with VBAO who have NIHSS scores of 10 or less at admission. This study aimed to develop and validate a clinical risk model based on clinical factors to predict prognosis in patients with wake-up stroke (WUS) after multimodal magnetic resonance imaging combined with recombinant tissue plasminogen activator intravenous thrombolysis. The study enrolled 263 patients with WUS, who were divided into the training (n = 162) and validation cohorts (n = 101). In the training cohort, patients were stratified based on modified Rankin Scale (mRS) score at 90 days after thrombolysis, with mRS ≤ 2 indicating a good prognosis (n = 117), and mRS > 2 indicating a poor prognosis (n = 45).

Materials And Methods: A retrospective review included 71 patients with 109 vertebral metastases who underwent microwave ablation combined with percutaneous vertebroplasty by the image-guided and real-time temperature monitoring. Treatment efficacy was determined by comparing visual analog scale (VAS) scores, daily morphine equivalent opioid consumption, and Oswestry Disability Index (ODI) scores before treatment and during the follow-up period.
