Two distinct types of systemic amyloidosis-transthyretin (ATTR) and light chain (AL)-account for the majority of cardiac amyloidosis (CA), which have been considered mutually exclusive. Diagnostic confusion can arise when both proteins are identified from an amyloid deposit. We present a rare occurrence of histopathologically confirmed concomitant ATTR and AL CA captured on 99mTc-3,3-diphosphono-1,2-propanodicarboxylic acid (DPD) scan and 18F-flutemetamol (FMM) PET/CT.

Background: Shortage of organ donors in the Republic of Korea has become a major problem. To address this, it has been questioned whether heart transplant (HTx) allocation should be modified to reduce priority of older patients. We aimed to evaluate post-HTx outcomes according to recipient age and specific pre-HTx conditions using a nationwide prospective cohort.

Aims: Hypertrophic cardiomyopathy (HCM) is associated with a significant risk of arrhythmia and heart failure (HF), yet treatment options for patients with HCM have remained limited. We aimed to investigate the relationship between sodium-glucose cotransporter-2 inhibitor (SGLT2i) use and clinical outcomes among patients with concurrent HCM and diabetes in real-world settings.

Methods And Results: Using the Korean National Health Insurance Service database, we identified patients with a confirmed diagnosis of HCM and prescriptions for antidiabetic drugs from 2018 to 2022.

Background: Despite the essential role of ectopic osteogenic calcium-phosphate metabolism in the development of calcific aortic valve disease (CAVD), the implications of high serum phosphate levels in CAVD development are not fully understood.

Methods: Asymptomatic individuals who underwent health screening using serial cardiac computed tomography (CT) and echocardiography were selected from a multicenter registry. CAVD was identified and quantified on CT images using the aortic valve calcification (AVC) score.

Background: Pulmonary thromboembolism and active haemoptysis represent distinct yet critical emergencies necessitating immediate intervention. However, the treatment protocols for these conditions-anticoagulation therapy and haemostatic therapy-often pose a dilemma.

Case Summary: We present the case of a 25-year-old female who presented to our emergency room with haemoptysis and a concurrent diagnosis of pulmonary thromboembolism.

Article Synopsis
  • This study explores the link between socioeconomic status (SES) and cardiovascular disease (CVD) mortality in cancer survivors, revealing that lower SES is associated with higher mortality rates.
  • Using a large database of cancer survivors, the research found that individuals in lower SES categories had progressively greater risks of CVD mortality, especially those with hypertension.
  • The results emphasize the need to consider SES as a critical risk factor for CVD in cancer survivors, particularly highlighting the combined impact of low SES and hypertension on mortality risks.

Background And Objectives: The number of people with heart failure (HF) is increasing worldwide, and the social burden is increasing as HF has high mortality and morbidity. We aimed to provide updated trends on the epidemiology of HF in Korea to shape future social measures against HF.

Methods: We used the National Health Information Database of the National Health Insurance Service to determine the prevalence, incidence, hospitalization rate, mortality rate, comorbidities, in-hospital mortality, and healthcare cost of patients with HF from 2002 to 2020 in Korea.

Article Synopsis
  • Current heart failure (HF) guidelines recommend a comprehensive approach for management, including patient education and self-care, but implementing these recommendations is difficult in practice.
  • A study was conducted to test a mobile health (mHealth) platform that pairs a smartphone app with Bluetooth monitoring devices to improve self-management and symptoms of heart failure.
  • Results showed that the intervention group reported significantly better symptom improvement and adherence to the app compared to the control group, demonstrating the potential benefits of technology in managing HF.

Article Synopsis
  • The prevention and management of cancer therapy-related cardiac dysfunction (CTRCD) are becoming increasingly crucial, especially as genetics play a significant role in a patient's susceptibility to this condition.
  • A case study of a 65-year-old woman with breast cancer illustrates the recurrence of CTRCD after low-dose chemotherapy, despite her not having typical heart risk factors, and indicates her genetic predisposition through findings of a variant in the TNNT2 gene.
  • The case emphasizes the potential benefits of genetic screening for cancer patients to identify those at risk for CTRCD, advocating for tailored chemotherapy plans and strategies to protect heart health during treatment.

Background: Post-transplant lymphoproliferative disorder (PTLD) is an important cause of morbidity and mortality in heart transplant (HTx) recipients. However, previous studies of PTLD after HTx are limited to single-center analyses or extrapolated from all solid organ transplantations.

Objectives: The authors analyzed the temporal trends, risk factors, and clinical outcome of de novo PTLD specifically after HTx.

Background/aims: Metabolic syndrome (MetS) raises the risk of cardiovascular disease and type 2 diabetes. An awareness of MetS is vital for early detection and proactive management, which can mitigate the risks associated with MetS. Therefore, our study aimed to investigate the level of awareness of MetS among the Korean population.

The Korean Society of Heart Failure (KSHF) Guidelines provide evidence-based recommendations based on Korean and international data to guide adequate diagnosis and management of heart failure (HF). Since introduction of 2017 edition of the guidelines, management of advanced HF has considerably improved, especially with advances in mechanical circulatory support and devices. The current guidelines addressed these improvements.

Most patients with heart failure (HF) have multiple comorbidities, which impact their quality of life, aggravate HF, and increase mortality. Cardiovascular comorbidities include systemic and pulmonary hypertension, ischemic and valvular heart diseases, and atrial fibrillation. Non-cardiovascular comorbidities include diabetes mellitus (DM), chronic kidney and pulmonary diseases, iron deficiency and anemia, and sleep apnea.

Article Synopsis
  • * Effective treatment for HF often includes a mix of medications like renin-angiotensin system inhibitors, diuretics, and anticoagulants based on specific conditions like atrial fibrillation or diabetes.
  • * Properly managing these comorbidities through medication and interventions like continuous positive airway pressure for sleep apnea is essential for better health outcomes in HF patients.

Background: F-sodium fluoride positron emission tomography/computed tomography (F-NaF PET/CT) has been proven to be useful in identification of microcalcifications, which are stimulated by inflammation. Blood speckle imaging (BSI) is a new imaging technology used for tracking the flow of blood cells using transesophageal echocardiography (TEE). We evaluated the relationship between turbulent flow identified by BSI and inflammatory activity of the aortic valve (AV) as indicated by the F-NaF uptake index in moderate aortic stenosis (AS) patients.

Background Hypertension is an important cause of morbidity, which predisposes patients to major cardiovascular events and mortality. The aim of this study was to explore the association between adherence to antihypertensive medication and clinical outcomes in adult patients with cancer. Methods and Results Using the 2002 to 2013 Korean National Health Insurance Service-National Sample Cohort, we extracted adult patients with cancer treated with antihypertensive medications.
