Background: Fulminant myocarditis (FM) is a potentially lethal disease with a wide spectrum of clinical presentation, thus making the diagnosis hard to depict. In cases where acute circulatory failure occurs venoarterial (VA) extracorporeal membrane oxygenation (ECMO) support is a valid management strategy, especially in the pediatric and adult patients. This study aims to report the results of VA ECMO for FM in our Institution.

Uncommon pathogens causing infective endocarditis.

Infective endocarditis is caused by a wide range of aetiological agents. The microbiology, epidemiology, and treatment of this disease have changed considerably in the last two decades. Staphylococci and streptococci are known to be the classical causative agents; however, blood culture-negative endocarditis caused by fastidious and slow-growing organisms is now common.

Sleep-disordered breathing (SDB), including obstructive and central sleep apnea, significantly exacerbates heart failure (HF) through adverse cardiovascular mechanisms. This review aims to synthesize existing literature to clarify the relationship between SDB and HF, focusing on the pathophysiological mechanisms, diagnostic challenges, and the effectiveness of treatment modalities like continuous positive airway pressure (CPAP) and adaptive servo-ventilation ASV. We analyzed peer-reviewed articles from 2003 to 2024 sourced from PubMed, EMBASE, Scopus, and Web of Science databases.

On algebraic naturalism and metaphysical indeterminacy in quantum mechanics.

I propose a technique for identifying fundamental properties using structures already present in physical theories. I argue that, in conjunction with a particular naturalistic commitment, that I dub 'algebraic naturalism', these structures can be used to generate a standard of metaphysical determinacy. This standard can be used to rule out the possibility of a virulent strain of 'deep' metaphysical indeterminacy that has been imputed to quantum mechanics.

Use of 16s rRNA Gene Sequencing for the Identification of Viridans Group Streptococci.

Streptococci belonging to the viridans group are gram-positive bacteria residing as commensals in the upper respiratory, gastrointestinal, and genital tracts in humans. Though they are largely known to be commensals, they may also cause life-threatening infections like infective endocarditis, septicemia, pyogenic infections, pneumonia, and meningitis. The viridans group streptococci (VGS) are usually identified by biotyping; however, species discrimination is not always possible by phenotypic characterization.

The coexistence of periodontal infections with coronary artery disease (CAD) has been shown in many studies. We investigated the presence and abundance of periodontitis-causing bacteria in patients with CAD.

Bowel Ischemia Secondary to Campylobacter Enterocolitis: A Case Series and Review of the Literature.

Campylobacter is the most common cause of diarrheal illness worldwide and is generally self-limiting. We present two cases of Campylobacter enterocolitis complicated by bowel ischemia in a 79-year-old male and a 53-year-old male, both presenting with abdominal pain and diarrhea and elevated lactate and C-reactive protein (CRP) levels. CT demonstrated common findings of pneumatosis intestinalis (PI) and portal venous gas.

Haemorrhagic mesenteric abscess in infective endocarditis: a unique cause of spontaneous haemoperitoneum.

Spontaneous haemoperitoneum (SH) is a rare but life-threatening condition with several well-recognized causes. SH often occurs in anticoagulated patients and requires urgent treatment to prevent mortality. SH is rarely associated with infective endocarditis (IE).

An Unusual Presentation of a Patient With Low-Dose Methotrexate Causing Colitis and Pancytopenia.

This paper summarizes the case of a patient admitted to the hospital due to methotrexate-induced colitis in the setting of pancytopenia. Although methotrexate is commonly used for a variety of diseases, this medication can precipitate side effects in a subset of patients. This is an atypical case of chronic low-dose methotrexate use leading to colitis and pancytopenia.
