Introduction: Emergency departments (EDs) have become the primary source of care for increasing number of patients, leading to treatment of nonemergent cases, which divert resources from true emergency situations and represent poor cost-effectiveness for treating such cases. There is evidence that suggests that patient navigation (PN) integrated into the ED and other case management techniques can help reduce the number of primary care-related ED visits and these navigation programs are more cost-effective than the ED visits themselves. The Greater New Orleans Community Health Connection Primary Care Capacity Project Quality Improvement Initiative (GNOPQii) is a pilot project aimed at improving the efficiency of PN for patients who have had avoidable ED encounters or inpatient readmissions through applied data and technology program.

The Community-Centered Health Home (CCHH) model, developed by Prevention Institute, calls on health care organizations to contribute to improvements in community conditions that influence health such as employment or housing. The Louisiana Public Health Institute designed the first demonstration of CCHH and worked with 5 community health centers in the Gulf South to implement the model over a 2-year period. With support of funding and technical assistance, health centers and their community partners pursued a range of data collection, analysis, and community prevention activities.

Vitamin A and its metabolite, retinoic acid (RA) are implicated in the regulation of immune homeostasis via the peripheral induction of regulatory T cells. Here we showed RA was also required to elicit proinflammatory CD4(+) helper T cell responses to infection and mucosal vaccination. Retinoic acid receptor alpha (RARα) was the critical mediator of these effects.
