Despite guideline recommendations, transfer rates to high-volume aortic centers (high-VACs) for acute type A aortic dissections (TAAD) remain suboptimal; this may be because the benefit of undergoing surgical repair of TAAD at high-VACs remains poorly quantified. Medicare beneficiaries undergoing surgical repair of TAAD from 1999-2019 were identified. Hospital and surgeon annual aortic case volumes in Medicare beneficiaries were determined.

Introduction: The heart team approach is now the standard of care for patients with complex coronary artery disease; however, the definition of a heart team is variable. We embarked on a project to create an extended, multidisciplinary heart team to evaluate patients we deemed high risk for coronary revascularization. In doing so, we created a new service, workflow, and paradigm.

Patients with newly diagnosed acute myeloid leukemia (ND-AML) derive variable survival benefit from venetoclax + hypomethylating agent (Ven-HMA) therapy. The primary objective in the current study was to develop genetic risk models that are predictive of survival and are applicable at the time of diagnosis and after establishing treatment response. Among 400 ND-AML patients treated with Ven-HMA at the Mayo Clinic, 247 (62%) achieved complete remission with (CR) or without (CRi) count recovery.

Type A aortic dissection (TAAD) has been associated with thoracic endovascular aortic repair (TEVAR) for aortic conditions: dissection and aneurysm. Our purpose was to study a subset of patients who had an initial TEVAR for type B aortic dissection, followed by a TAAD, which was treated by open ascending aortic repair. Over a 6-year period, 171 consecutive patients underwent open ascending aortic repair for TAAD, of whom 21 (12%) had a previous cardiovascular procedure, 17 of which were endovascular.

Key Points: CKD-associated pruritus is highly prevalent among peritoneal dialysis patients. Pruritus is associated with reduced health-related quality of life, and the composite of mortality and transfer to hemodialysis for peritoneal dialysis patients. Efforts to better identify and manage pruritus in this population are needed.

The transcription factor MYT1L supports proper neuronal differentiation and maturation during brain development. MYT1L haploinsufficiency results in a neurodevelopmental disorder characterized by intellectual disability, developmental delay, autism, behavioral disruptions, aggression, obesity and epilepsy. While MYT1L is expressed throughout the brain, how it supports proper neuronal function in distinct regions has not been assessed.

A congenitally malformed aortic valve, unicuspid (UAV), or bicuspid (BAV), occurs in about 1% of the population and is known to be more frequent in patients with aortic dissection. The clinical and operative findings in a series of 134 patients with spontaneous, acute type A aortic dissection were studied, comparing patients with normal and abnormal aortic valve morphology. The aortic valve was normal in 123 of 134 (92%) patients and abnormal in 11 of 134 (8%) patients: BAV in 10 of 134 (7.

Diets that are high in fat cause over-eating and weight gain in multiple species of animals, suggesting that high dietary fat is sufficient to cause obesity. However, high-fat diets are typically provided freely to animals in obesity experiments, so it remains unclear if high-fat diets would still cause obesity if they required more effort to obtain. We hypothesized that unrestricted and easy access is necessary for high-fat diet induced over-eating, and the corollary that requiring mice to perform small amounts of work to obtain high-fat diet would reduce high-fat diet intake and associated weight gain.

Background: Although the Impella device has an established role in high-risk percutaneous intervention and cardiogenic shock, its role in open cardiac surgery remains unclear. We undertook this study to better understand the role of Impella support in cardiac surgical intervention.

Methods: This is a retrospective cohort study of consecutive patients who underwent cardiac surgery with surgically placed Impella 5.

Article Synopsis
  • LVAD implantation significantly improves patient functional status, as shown by the New York Heart Association (NYHA) functional class, with most patients showing improvement at both 6-month and 12-month follow-ups.
  • A retrospective study of 151 patients found that 92.62% improved at 6 months, with many achieving class I or II symptoms, and this improvement remained stable over 2 years.
  • The study identified ischemic heart failure as a strong predictor of sustainable functional improvement following LVAD implantation, highlighting the potential benefits for high-risk patients.

Article Synopsis
  • - Pruritus, or itchiness, is a common issue affecting 43% of peritoneal dialysis patients, with prevalence varying by country, being notably higher in Thailand (50%) and lower in the U.S. (33%).
  • - Factors like diabetes, low protein levels, and high phosphorus levels are linked to moderate to severe pruritus, while increased urine output appears to reduce itchiness.
  • - Patients suffering from severe pruritus report poorer mental and physical health, and they also have a higher risk of death or needing to switch to hemodialysis (HD), with a 12% increased risk associated with moderate to extreme itching.

Article Synopsis
  • Individuals can experience long COVID symptoms even after having mild or no symptoms during the acute phase of infection, highlighting the need for accurate public health assessments.
  • This study analyzed over 10 million confirmed COVID-19 cases in California to estimate the loss of quality-adjusted life years (QALYs) due to long COVID across different age groups, finding that younger adults (18-49) faced the highest overall loss.
  • The analysis suggests that while older adults experience severe long COVID effects, younger individuals may suffer significant cumulative impacts from mild symptoms, emphasizing the importance of understanding long COVID across all age groups.

Background: More than four years after the start of the COVID-19 pandemic, understanding of SARS-CoV-2 burden and post-acute sequela of COVID (PASC), or long COVID, continues to evolve. However, prevalence estimates are disparate and uncertain. Leveraging survey responses from a large serosurveillance study, we assess prevalence estimates using five different long COVID definitions among California residents.

Objective: Long-term outcomes after multivalve cardiac surgery remain underevaluated.

Methods: Medicare administrative claims from 2008 to 2019 identified beneficiaries undergoing multivalve surgery. Operative characteristics were doubly adjudicated using International Classification of Diseases and Current Procedural Technology codes.

The LAL test has been the "gold standard" for endotoxin testing ever since it was first published as USP Chapter <85> in 1980. Since then, a number of innovative methods have been proposed to augment or replace the LAL test. However novel an alternate test method might be, we must be cognizant of our obligation to patient safety and heed compendial and compliance requirements to demonstrate results equivalence between any candidate (alternate) method and a predicate (compendial) test method.

Article Synopsis
  • The study introduces a new system called A-B-S2EBPR for efficient nitrogen removal from low-strength municipal wastewater by promoting nitrite accumulation.
  • Researchers achieved significant nitrite levels (5.5 mg N/L) and high removal efficiency of total inorganic nitrogen (TIN at 84.9%) through a combination of biological processes.
  • The findings highlight a shift in microbial communities that favor partial denitrification over conventional nitrification, with specific microorganisms utilizing carbon sources during the process.
