Article Synopsis
  • SGLT2 inhibitors like empagliflozin can improve heart failure outcomes in type 2 diabetes, but how they work is still not well understood.
  • A study involving 90 patients with coronary artery disease and type 2 diabetes assessed the effects of empagliflozin on left atrial (LA) volume and function over 6 months.
  • The results showed no significant changes in LA volume or function between the empagliflozin group and the placebo group, indicating that the drug may not impact these specific heart aspects.

  • Patient reported outcomes (PROs) are vital for assessing health-related quality of life in adult patients with congenital heart disease, but they are not well defined for this population.
  • This study aimed to evaluate how well different PRO survey tools could distinguish between patients with Fontan circulatory failure (FCF) and those with simpler shunt lesions.
  • The Kansas City Cardiomyopathy Questionnaire (KCCQ-12) showed the best ability to identify FCF, and combining it with other PRO tools further improved its effectiveness, suggesting that these measures can help inform management strategies for adult congenital heart disease patients.

Article Synopsis
  • The study aims to evaluate how the use of pulmonary artery catheters (PACs) during cardiac surgery affects patient outcomes in the hospital.
  • It analyzed data from seven studies involving over 25,000 patients, finding that PAC use is linked to a higher risk of in-hospital mortality and longer stays in intensive care.
  • The findings indicate a clear need for further research, particularly randomized trials, to better understand the impact of PAC on cardiac surgery patients' health outcomes.

Sodium-glucose cotransporters 2 (SGLT2) are high-capacity, low-affinity transporters, expressed mainly in the early portion of the proximal renal tube, mediating up to 90% of renal glucose uptake, while SGLT1 receptors are found mainly in the small intestine, facilitating glucose absorption. SGLT2 inhibitors (SGLT2i) originally emerged as agents for the treatment of type 2 diabetes mellitus; however, they soon demonstrated remarkable cardio- and renoprotective actions that led to their licensed use for the treatment of heart failure and chronic kidney disease, regardless of the diabetic status. Cardiovascular remodelling represents an umbrella term that encompasses changes that occur in the cardiovascular system, from the molecular and cellular level, to tissue and organs after local injury, chronic stress, or pressure.

Article Synopsis
  • - REDUCE-IT trial showed icosapent ethyl (IPE) decreased major cardiovascular events by 25%, but the exact reasons for its benefits were unclear, leading to the IPE-PREVENTION CardioLink-14 trial to explore its effect on vascular regenerative (VR) cell content in individuals with high triglycerides.
  • - In the study, 70 individuals on statins were given either IPE (4 g/day) or usual care; results showed IPE increased the frequency of ALDHSSC CD133 progenitor cells and reduced oxidative stress in progenitor cells, even as overall ALDHSSC cell frequency decreased.
  • - The findings suggest that IPE not only impacts VR cell content but may also

Anemia and renal failure are independent risk factors for perioperative stroke, prompting us to assess the combined impact of acute hemodilutional anemia and bilateral nephrectomy (2Nx) on microvascular brain Po (Po) in a rat model. Changes in Po (phosphorescence quenching) and cardiac output (CO, echocardiography) were measured in different groups of anesthetized Sprague-Dawley rats (1.5% isoflurane, = 5-8/group) randomized to Sham 2Nx or 2Nx and subsequently exposed to acute hemodilutional anemia (50% estimated blood volume exchange with 6% hydroxyethyl starch) or time-based controls (no hemodilution).

Article Synopsis
  • South Asian individuals have a higher prevalence of cardiometabolic diseases compared to White Europeans, prompting a study on their vascular regenerative cell content.
  • The study included 60 South Asians and 60 White Europeans, revealing that South Asians had lower levels of key regenerative cells and exhibited more severe diabetes symptoms.
  • Findings suggest that South Asians possess compromised vascular repair capabilities, which may explain why they face greater cardiovascular risks.

Modulation of the renin-angiotensin-aldosterone system is a foundation of therapy for cardiovascular and kidney diseases. Excess aldosterone plays an important role in cardiovascular disease, contributing to inflammation, fibrosis, and dysfunction in the heart, kidneys, and vasculature through both genomic and mineralocorticoid receptor (MR)-mediated as well as nongenomic mechanisms. MR antagonists have been a key therapy for attenuating the pathologic effects of aldosterone but are associated with some side effects and may not always adequately attenuate the nongenomic effects of aldosterone.

Background: Recent data from the randomized SUSTAIN CSX trial could not confirm clinical benefits from perioperative selenium treatment in high-risk cardiac surgery patients. Underlying reasons may involve inadequate biosynthesis of glutathione peroxidase (GPx3), which is a key mediator of selenium's antioxidant effects. This secondary analysis aimed to identify patients with an increase in GPx3 activity following selenium treatment.

Anemia and red blood cell transfusion are associated with adverse outcomes. Large randomized trials have demonstrated that restrictive transfusion strategies can safely reduce patient exposure to transfusion, forming a foundational pillar of patient blood management. The restrictive transfusion thresholds in these trials ranged from 7 to 8 g/dL for different populations and this variability is reflected in recent guideline recommendations for transfusion triggers.

Background: This exploratory sub-analysis of the EMPA-HEART CardioLink-6 trial examined whether the previously reported benefit of the sodium-glucose cotransporter 2 (SGLT2) inhibitor empagliflozin on left ventricular (LV) mass (LVM) regression differs between individuals of South Asian and non-South Asian ethnicity.

Methods: EMPA-HEART CardioLink-6 was a double-blind, placebo-controlled clinical trial that randomised 97 individuals with type 2 diabetes mellitus (T2DM) and coronary artery disease (CAD) to either empagliflozin 10 mg daily or placebo for 6 months. LV parameters and function were assessed using cardiac magnetic resonance imaging.

Although the role of inflammation in atherosclerosis is established, whether this relationship remains important after lowering low-density lipoprotein cholesterol (LDL-C) is still unclear. Recently in The Lancet, Ridker et al. demonstrated that residual inflammatory risk was a stronger predictor of fatal and non-fatal events compared to residual cholesterol risk, supporting the concept of inflammation testing to guide vascular risk reduction.

Background: The cardiovascular (CV) benefits of sodium-glucose transport protein 2 inhibitors have been attributed, in part, to cardiac reverse remodelling. The EMPA-HEART CardioLink-6 study reported that sodium-glucose cotransporter-2 inhibition for 6 months with empagliflozin was associated with a significant reduction in left ventricular mass indexed to body surface area (LVMi). In this sub-analysis, we evaluated whether baseline LVMi may influence how empagliflozin affects cardiac reverse remodelling.
