Article Synopsis
  • This study examined how two levels of low-intensity vibration training (0.6 g and 1.0 g) affected muscle and bone recovery in mice after a muscle injury known as volumetric muscle loss (VML).
  • Mice were divided into groups receiving either no treatment or varying levels of vibration training for 15 minutes a day over 8 weeks post-injury.
  • Results showed that bone structure improved with vibration training, particularly at the 1.0 g level, while muscle function showed less significant enhancement, suggesting that vibration training may be more beneficial for bone recovery than for muscle after VML.

Background: Succinylcholine and rocuronium are the most commonly utilized neuromuscular blocker agents (NMBAs) for rapid sequence intubation (RSI) in the emergency department (ED). The duration of action of rocuronium is significantly longer (∼30 min) compared to succinylcholine (∼10 min) and previous studies have shown that patients receiving rocuronium are more likely to have longer time to sedation initiation following RSI. Furthermore, patients receiving rocuronium may be more likely to experience awareness with paralysis than those receiving succinylcholine.

Background: Children with single-ventricle congenital heart disease typically undergo a superior cavopulmonary connection (SCPC) as the second stage in their surgical palliation. Postoperatively, stenoses of the SCPC and branch pulmonary arteries can occur. If there are clinical concerns and echocardiography is insufficient for diagnosis, patients undergo invasive evaluation with exposure to radiation and anesthesia.

Article Synopsis
  • Obstructed total anomalous pulmonary venous connection (TAPVC) is a critical heart defect that requires urgent care after birth, and understanding which patients have severe obstruction can help in planning deliveries.
  • The study analyzed 29 neonates diagnosed prenatally with TAPVC, focusing on a new measurement called the pulmonary venous variability index (PVVI) and Doppler velocities of a vertical vein to predict postnatal obstruction severity.
  • Results showed that lower PVVI values and specific vertical vein velocities were linked to a higher risk of severe pulmonary vein obstruction after birth, indicating these metrics could be valuable for managing affected infants.

Background: While numerous indices have been developed in an attempt to quantify ventricular function in patients with single ventricle heart disease after Fontan, there are little data on how these parameters change over time.

Methods: A retrospective observational study was performed of individuals who underwent Fontan operation at Children's Hospital of Philadelphia (CHOP) in 2006 and 2007. Measurements of fractional area change (FAC), tricuspid annular planar systolic excursion (TAPSE), myocardial performance index (MPI), systolic to diastolic (s/d) ratio, and myocardial strain and strain rate were made offline.

This study evaluates the effectiveness of mandatory pulse oximetry screening. The objective is to evaluate whether mandatory pulse oximetry testing had decreased the late critical congenital heart disease (CCHD) diagnosis rate and reduced mortality in neonatal subjects. This was a single center, retrospective cohort study comparing the timing of diagnosis of CCHD between neonates undergoing cardiac surgery in 2009-2010, prior to mandatory pulse oximetry screening, and neonates in 2015-2016, after mandatory pulse oximetry screening was instituted.

Background: Hypertrophic cardiomyopathy (HCM) is the leading cause of sudden cardiac death in athletes. However, preparticipation electrocardiogram (ECG) screening has been criticized for failing to meet cost-effectiveness thresholds, in part because of high false-positive rates. We sought to evaluate whether a highly automated software algorithm could be used for a high throughput, population-based screening program and address several of the limitations seen with population-based screening

Methods: A proprietary computed algorithm was created based on both voltage- as well as Seattle-based ECG criteria.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a highly malignant genetic channelopathy associated with exertional syncope and reproducible polymorphic ventricular tachycardia with exercise. Approximately 65% of patients with CPVT are found to have a disease causing mutation in the RYR2 gene. RYR2 encodes a calcium ion transporter in the sarcomeric reticulum, and is responsible for the calcium induced calcium release that results in ventricular contraction.

A racemic Au(I)-catalyzed three-component reaction has been developed to prepare cyclic carbamimidates from imines, terminal alkynes, and sulfonylisocyanates. This reaction exploits the carbophilic π-acidity of gold catalysts to first activate an alkyne toward deprotonation and secondly, to activate the internal alkyne generated toward intramolecular O-cyclization. Unlike similar previously reported multicomponent gold-catalyzed reactions, the stereocenter generated during the alkynylation is preserved in the product.

This Perspective details a developing research program that emerged from simple plans for achieving the synthesis of tetrahydrofurans from cyclopropanes and C═O π bonds. Lewis acid catalyzed annulations of malonate-derived donor-acceptor cyclopropanes with aldehydes are unusually broad in scope and lead to the synthesis of structurally diverse tetrahydrofurans. The reactions are stereospecific, with inversion observed at the cyclopropane donor site.

Recent evidence suggests that blocking aberrant hedgehog pathway signaling may be a promising therapeutic strategy for the treatment of several types of cancer. Cyclopamine, a plant Veratrum alkaloid, is a natural product antagonist of the hedgehog pathway. In a previous report, a seven-membered D-ring semisynthetic analogue of cyclopamine, IPI-269609 (2), was shown to have greater acid stability and better aqueous solubility compared to cyclopamine.
