Background: Heart failure (HF) is the leading cause of mortality, morbidity, and rehospitalization in Appalachia. Rural areas have the highest HF mortality rates. Rural Appalachians lack access to health services and end-of-life palliative care (EOLPC) and have extreme inequities in health.

Purpose: The purpose of this brief was to describe the development of the concept transcending health values and its application among the conservative Anabaptist community.

Method: This phenomenon was developed using an established 10- phase concept building process. Initially, a practice story emerged from an encounter that led to the development of the concept and core qualities.
