Annotating genomic sequence alterations is sometimes a difficult decision, particularly in missense variants with uncertain pathogenic significance and also in those presumed as germline pathogenic variants. We here suggest that mutation spectrum may also be useful for judging them. From the public databases, 982 BRCA1/1861 BRCA2 germline missense variants and 294 BRCA1/420 BRCA2 somatic missense variants were obtained.

  • Flexible laryngoscopy (FL) is the standard method for diagnosing vocal cord paralysis (VCP), but it can be uncomfortable for patients; dynamic digital radiography (DDR) offers a less invasive alternative with better ease of use.
  • A study involving 78 patients showed that DDR had 67% sensitivity and 100% specificity for diagnosing VCP, with a critical vocal cord movement (VCM) cutoff of 2.4 mm.
  • DDR showed promise for diagnosing postoperative VCP and predicting which patients might develop permanent VCP, with impressive results regarding the detection of transient VCP.

Article Synopsis
  • - A 52-year-old man with Stage Ⅲc colon cancer had surgery and chemotherapy, showing no recurrence for 7 years until left lung metastasis was found and removed.
  • - Two years later, mesenteric lymph node metastasis was discovered, leading to further surgery, but subsequent attempts to remove mediastinal lymph nodes failed due to their strong adhesion to the trachea.
  • - Despite receiving chemotherapy and radiation therapy, the patient's condition worsened, resulting in significant complications, and he passed away 4 months after restarting treatment, highlighting the rarity and challenges of managing late-stage recurrence after colon cancer.

Background: Dynamic chest radiography (DCR) produces sequential radiographs within a short examination time. It is also inexpensive and only uses a low dose of radiation. Because of the lack of reports of evaluating cardiac function using DCR in humans, we investigated its discriminative ability for left ventricular (LV) dysfunction in a study cohort.

We investigated the usefulness of circulating miR-216a-5p and miR-217-5p that are pancreas-enriched micro RNAs (miRNAs) as biomarkers of acute pancreatic damage, and compared them with conventional pancreatic biomarkers in L-arginine-induced acute pancreatitis mouse model. As the results, amylase and lipase levels apparently increased and peaked on Day 3 when acute pancreatitis including acinar cell degeneration/necrosis and inflammatory cell infiltration reached its peak. In contrast, miR-216a-5p and miR-217-5p increased from Day 1 when histopathological findings in the acinar cells were limited to decreased zymogen granules, and the increases in ratios were much higher than those of amylase and lipase.

Background: Renal cell carcinoma (RCC) and non-small cell lung cancer (NSCLC) are representative malignancies that respond well to immune checkpoint inhibitors (ICIs). Research has been conducted to identify biomarkers, such as programmed death ligand-1 (PD-L1), that would allow the response to ICI therapy to be predicted; however, the complex tumor immune system consisting of both host and tumor factors may also exert an influence.

Case Presentation: Computed tomographic imaging (CT) incidentally revealed a left renal mass, and a left pulmonary nodule with multiple lymph node metastases (LNMs).

Local control is important in the treatment of locally advanced rectal cancer(LARC). In Western countries, the standard therapy for LARC is preoperative chemoradiotherapy(CRT)followed by total mesorectal excision. Recently, addition of intensive chemotherapy to CRT known as total neoadjuvant therapy(TNT)is increasing.

Background: The standard of care for unresectable, locally advanced non-small-cell lung cancer (LA-NSCLC) is chemoradiotherapy (CRT) followed by durvalumab, based on the PACIFIC study. Although multiple Japanese phase II studies have shown high efficacy and tolerability of CRT with cisplatin plus S-1 (SP), no prospective study using durvalumab after SP-based CRT has been reported.

Objectives: We conducted a multicenter phase II study of this approach, the interim analysis of which showed a high transition rate to durvalumab consolidation therapy.

Article Synopsis
  • - The study aimed to assess the impact of combining one-lung ventilation with carbon dioxide insufflation (OLV-CDI) on the working space available during thoracoscopy in Beagle dogs, using CT scans for evaluation.
  • - Six healthy Beagles underwent procedures with variations of OLV and OLV-CDI at different intrapleural pressure levels, revealing that OLV-CDI significantly increased intrathoracic working space, especially at 3 mm Hg, compared to OLV alone.
  • - While OLV-CDI improved working space, higher pressures (5 mm Hg) reduced ventilation space and oxygen levels, causing adverse effects like pneumothorax in some dogs, indicating that more research is needed before applying this
