Article Synopsis
  • The study aimed to see if adding oral glucocorticoids to immunosuppressive therapy improves skin conditions and safety in patients with early diffuse cutaneous systemic sclerosis (dcSSc).
  • It compared two groups: one receiving glucocorticoids combined with immunosuppression, and the other receiving only immunosuppression, assessing changes in the modified Rodnan skin score (mRSS) over about a year.
  • Results showed no significant difference in skin score improvement between the groups, indicating that low-dose glucocorticoids didn't provide added benefits for skin fibrosis nor raised the risk of scleroderma renal crisis.

Background: Psoriatic arthritis (PsA) is a chronic, immune-mediated, spondyloarthropathy characterised by musculoskeletal signs and symptoms with associated joint pain and tenderness. The average worldwide PsA prevalence is 133/100,000, while in the Italian population is 90-420/100,000. Traditionally, nonsteroidal anti-inflammatory drugs, glucocorticoid, and disease-modifying antirheumatic drugs have been used in the treatment of PsA.

Article Synopsis
  • Biologics have shown efficacy in treating Psoriatic Arthritis (PsA) in clinical trials, but more real-world evidence is needed to confirm their effectiveness in everyday clinical settings.
  • The CHRONOS study was conducted in 20 Italian hospitals and included 399 patients, revealing that the most common biologic used was secukinumab, with a 71.8% response rate after 6 months.
  • Results demonstrated significant improvements in secondary outcomes such as skin symptoms and joint inflammation, supporting the effectiveness of biologics for PsA in practical applications, aligning with previous trial findings.

Phenotypic features and outcome differences between sexes have been reported in psoriatic arthritis (PsA). However, little is known about sex differences in effectiveness of biologics in clinical practice. gender analysis of the CHRONOS, a multicenter, noninterventional, retroprospective Italian real-world study assessing 6-month and 1-year effectiveness of biologics for PsA.

Objectives: Baricitinib, an oral Janus kinase (JAK) 1-2 inhibitor, is currently used along biologic DMARDs (bDMARDs) after the failure of methotrexate (MTX) in rheumatoid arthritis (RA). We investigated the efficacy and safety of baricitinib in real life.

Methods: We prospectively enrolled 446 RA patients treated with baricitinib from 11 Italian centres.

Antisynthetase syndrome (ASSD) is a rare clinical condition that is characterized by the occurrence of a classic clinical triad, encompassing myositis, arthritis, and interstitial lung disease (ILD), along with specific autoantibodies that are addressed to different aminoacyl tRNA synthetases (ARS). Until now, it has been unknown whether the presence of a different ARS might affect the clinical presentation, evolution, and outcome of ASSD. In this study, we retrospectively recorded the time of onset, characteristics, clustering of triad findings, and survival of 828 ASSD patients (593 anti-Jo1, 95 anti-PL7, 84 anti-PL12, 38 anti-EJ, and 18 anti-OJ), referring to AENEAS (American and European NEtwork of Antisynthetase Syndrome) collaborative group's cohort.

This comprehensive review summarizes retrospective and prospective studies on pregnancy in systemic sclerosis in order to educate physicians on critical management issues. Fertility is normal in women with established systemic sclerosis. Their rates of spontaneous losses are comparable to the general population, except for patients with late diffuse systemic sclerosis and severe internal organ involvement who may have higher risks of abortion.

Objective: To assess fetal and maternal outcomes in women with systemic sclerosis (SSc).

Methods: Prospectively collected data on 99 women with SSc from 25 Italian centers were analyzed retrospectively. Women with SSc were observed during 109 pregnancies (from 2000 to 2011), and outcomes were compared to those in the general obstetric population (total of 3,939 deliveries).
