Background: It is known that the mortality of pneumonia in patients with risk factors for aspiration is worse than that in those without these risk factors. However, it is still unknown which risk factors for aspiration predict prognosis. Therefore, we aimed to determine which risk factors for aspiration are associated with death or prolonged hospitalization.

Background: Nontuberculous mycobacterial pulmonary disease (NTM-PD) often exhibits pulmonary function impairment, such as obstructive or restrictive pattern, with variation among patients according to the damaged lesions in the lung.

Methods: Patients with NTM-PD were consecutively enrolled between September 2019 and December 2020 at the Respiratory Infection Clinic of our hospital. Patients' data were comprehensively collected through laboratory examinations, PFT, chest computed tomography, and questionnaires for the assessment of subjective symptoms and health-related quality of life (HRQOL).

Article Synopsis
  • The study investigates whether the distribution of ground-glass opacity (GGO) and consolidation in the lungs of patients with SARS-CoV-2 pneumonia correlates with severe health outcomes.
  • Researchers employed AI-driven image analysis on CT scans of 512 hospitalized patients from January 2020 to August 2021 to measure these distributions.
  • Results indicated that a higher central-peripheral ratio of GGO was significantly associated with severe outcomes, suggesting that analyzing lung opacity patterns can aid in identifying high-risk COVID-19 patients.

Article Synopsis
  • Research investigates the impact of inhaled corticosteroids (ICS) and biologics on the immune response to mRNA COVID-19 vaccination in asthma patients.
  • The study involved 30 patients, measuring immune markers at 3 weeks and 6 months after their second vaccine dose, finding no significant differences between those on biologics and those not.
  • Key factors like age and sex affected immune response, but ICS and biologics didn't seem to influence vaccine effectiveness; further research is needed for confirmation.

Pulmonary and extrapulmonary complications after coronavirus disease 2019 (COVID-19) have been major public health concerns during the COVID-19 pandemic. Although post-COVID-19 pulmonary manifestations cover a wide spectrum, eosinophilic pneumonia (EP) has rarely been reported. To date, only four cases of EP potentially triggered by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection have been reported, all of which required systemic corticosteroid therapy.

The aim of this study was to longitudinally evaluate the undetermined impact of methotrexate (MTX) on the cumulative immunogenicity elicited by three doses of SARS-CoV-2 mRNA vaccination in patients with rheumatoid arthritis (RA). We prospectively evaluated vaccine-induced immune responses following the first dose, 1 and 6 months after the second dose, and 1 month after the third dose of BNT162b2 or mRNA-1273 in 144 SARS-CoV-2 naïve participants (70 patients with RA, 29 disease controls without immunosuppressive conditions, and 45 healthy controls). Humoral immune responses were assessed by quantifying anti-spike IgG antibody titers and the capacity of circulating antibodies to neutralize the ancestral SARS-CoV-2 strain and the Alpha, Delta, and Omicron variants.

Background: Influenza-related bacterial pneumonia is a leading complication of influenza infection. However, the differences in the incidence rates and risk factors associated with concomitant viral/bacterial pneumonia (CP) and secondary bacterial pneumonia following influenza (SP) remain unclear. This study aimed to clarify the incidence rates of CP and SP following seasonal influenza and identify factors associated with their development.

Introduction: Despite the poor prognosis for nursing home acquired pneumonia (NHAP), a useful prognostic factor is lacking. We evaluated protein C (PC) activity as a predictor of in-hospital death in patients with NHAP and community-acquired pneumonia (CAP).

Methods: This prospective, observational study included all patients hospitalized with pneumonia between July 2007 and December 2012 in a single hospital.

Purpose: An episodic increase in transcutaneous carbon dioxide pressure (PtcCO) is often recognized in patients with advanced chronic obstructive pulmonary disease (COPD) by overnight PtcCO monitoring. This phenomenon, called episodic nocturnal hypercapnia (eNH), mainly corresponds to rapid eye movement (REM) sleep-related hypoventilation. However, it is unclear whether eNH is associated with the frequency of COPD exacerbation.

The discriminative power of CURB-65 for mortality in community-acquired pneumonia (CAP) is suspected to decrease with age. However, a useful prognostic prediction model for older patients with CAP has not been established. This study aimed to develop and validate a new scoring system for predicting mortality in older patients with CAP.

Background: Detailed differences in clinical information between severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pneumonia (CP), which is the main phenotype of SARS-CoV-2 disease, and influenza pneumonia (IP) are still unclear.

Methods: A prospective, multicenter cohort study was conducted by including patients with CP who were hospitalized between January and June 2020 and a retrospective cohort of patients with IP hospitalized from 2009 to 2020. We compared the clinical presentations and studied the prognostic factors of CP and IP.

Introduction: Risk factors associated with the new detection of methicillin-resistant Staphylococcus aureus (MRSA) during hospitalization remain unclear. This study aimed to identify risk factors associated with MRSA isolation from the sputum of patients admitted with pneumonia, during their hospitalization.

Methods: Patients were prospectively enrolled from 2003 to 2012.

Article Synopsis
  • This study compares the effectiveness of two first-line antibiotics, ceftriaxone (CTRX) and ampicillin/sulbactam (ABPC/SBT), for treating non-aspiration community-acquired pneumonia (CAP) in adults.
  • A total of 263 patients were involved, showing similar clinical effectiveness rates at the end of treatment (90% for CTRX and 96% for ABPC/SBT), but ABPC/SBT proved to be significantly more effective by day 7.
  • While the overall outcomes at the end of the study did not show significant differences between the two groups, ABPC/SBT had a higher cure rate and lower relapse and failure rates.
