One-stage revision anterior cruciate ligament reconstruction: Preoperative evaluation, planning and surgical techniques. A review of current concepts.

Unlabelled: The increased rate of anterior cruciate ligament (ACL) tears has led to a greater number of revisions. Revision surgery can be performed in one or two stages. Single-stage revision ACL reconstruction (ssRACLR) may be performed when prior tunnels can be re-used or bypassed whereas a two-stage procedure is indicated when bone grafting of dilated tunnels prior to revision is necessary.

Objectives: Septic arthritis after anterior cruciate ligament (ACL) reconstruction is a rare but potentially devastating complication. The imaging findings associated with such infections are not well-described or quantified. The purpose of this study was to describe and quantify the frequency of the characteristic MRI findings of infection following ACL reconstruction.

Reassurance for Patients-Essential Not Optional.

Appropriate patient reassurance is an essential feature of clinical practice. My recent experience as a patient, interpreted via my expertise as a health services researcher, led me to insights on ideal and suboptimal reassurance styles in the context of worrisome symptoms. Reassurance is complex: often poorly defined in the scientific literature, rarely rigorously studied, imperfectly understood, and requiring some adaptation to each patient situation.

Considerations Regarding High Oil Density Bioreactor-Scale Fermentations of Yarrowia lipolytica Using CFD Modeling and Experimental Validation.

Hydrophobic feedstocks such as waste cooking oil have recently been considered for microbial biotransformation due to their abundance, low cost, and unique advantage for lipid-derived fermentation products. Most fermentations with hydrophobic substrates are conducted at the tube or flask scale (less than 1 L total volume) or with the hydrophobic substrate comprising a small fraction of the media. Low substrate concentrations require additional feeding.

High-Statistics Measurement of the Cosmic-Ray Electron Spectrum with H.E.S.S.

Owing to their rapid cooling rate and hence loss-limited propagation distance, cosmic-ray electrons and positrons (CRe) at very high energies probe local cosmic-ray accelerators and provide constraints on exotic production mechanisms such as annihilation of dark matter particles. We present a high-statistics measurement of the spectrum of CRe candidate events from 0.3 to 40 TeV with the High Energy Stereoscopic System, covering 2 orders of magnitude in energy and reaching a proton rejection power of better than 10^{4}.

Youth experiences in gender-sexuality alliances predict academic engagement but not disaffection through social-emotional wellbeing.

Experiences in gender-sexuality alliances (GSAs) could predict youth's academic engagement through improved social-emotional wellbeing (indicated by school belonging, hope, and positive and negative affect). This study utilized three waves of data, each spaced 2-3 months apart, among 627 youth (87% LGBQ+, 45% trans/nonbinary, 48% youth of color) ages 11-22 (M = 15.13) in 51 GSAs in the United States in 2021-2022.

A Novel Machine Learning Model to Predict Revision ACL Reconstruction Failure in the MARS Cohort.

Background: As machine learning becomes increasingly utilized in orthopaedic clinical research, the application of machine learning methodology to cohort data from the Multicenter ACL Revision Study (MARS) presents a valuable opportunity to translate data into patient-specific insights.

Surgical Predictors of Clinical Outcome 6 Years After Revision ACL Reconstruction.

Background: Revision anterior cruciate ligament (ACL) reconstruction has been documented to have inferior outcomes compared with primary ACL reconstruction. The reasons why remain unknown.

Purpose: To determine whether surgical factors performed at the time of revision ACL reconstruction can influence a patient's outcome at 6-year follow-up.

Development of a Single Vial Mass Flow Rate Monitor to Assess Pharmaceutical Freeze Drying Heterogeneity.

During pharmaceutical lyophilization processes, inter-vial drying heterogeneity remains a significant obstacle. Due to differences in heat and mass transfer based on vial position within the freeze drier, edge vials freeze differently, are typically warmer and dry faster than center vials. This vial position-dependent heterogeneity within the freeze dryer leads to tradeoffs during process development.

Editorial Commentary: Anterior Cruciate Ligament Tear Treatment Requires Consideration of Physical Examination, Graft Selection, Surgical Technique, Rehabilitation, and Expected Outcomes With a Primary Goal of Prevention of Failure or Revision Surgery.

Appropriate graft sizing is imperative for the success of anterior cruciate ligament reconstruction and is more challenging for hamstring autografts than grafts with bone plugs. It is important to select the optimal graft for each patient on the basis of their characteristics. When researching anterior cruciate ligament reconstruction outcomes, we prefer a holistic approach, assessing many variables, including appropriate graft selection and sizing, surgical technique, physical examination, rehabilitation milestones, patient-reported outcomes, and return to sport, to view the whole picture.

2024 Kappa Delta Ann Doner Vaughan Award: Nonsurgical Treatment of Symptomatic, Atraumatic Full-Thickness Rotator Cuff Tears-a Prospective Multicenter Cohort Study With 10-Year Follow-Up.

The Multicenter Orthopaedic Outcomes Network Shoulder Group conducted a prospective cohort study of 452 patients with symptomatic atraumatic rotator cuff tears treated with a physical therapy program to determine the predictors of failure of nonsurgical treatment, to provide insight into indications for surgery. After 10 years, we found the following: (1) Physical therapy was effective for over 70% of patients. (2) PROMs showed statistical and clinical improvement after 12 weeks of therapy and did not decline over 10 years.

Anatomic Fibular-Based Posterolateral Corner Reconstruction With 2 Femoral Tunnels Shows Lowest Residual Laxity With External Rotation and Varus Stresses: A Systematic Review and Network Meta-analysis of In Vitro Biomechanical Studies.

Purpose: To perform a systematic review and network meta-analysis of in vitro cadaveric, biomechanical studies evaluating described techniques for posterolateral corner (PLC) reconstruction, including fibular- and tibiofibular-based techniques.

Methods: The PubMed/MEDLINE, Embase, and Cochrane Library databases were searched in December 2023 for cadaveric studies evaluating PLC reconstruction. After a descriptive summary, a series of frequentist network meta-analyses comparing (1) nonanatomic fibular-based (single femoral tunnel), (2) anatomic fibular-based (double femoral tunnel), and (3) anatomic tibiofibular-based PLC reconstructions with the intact knee were performed for both external rotation (ER) and varus laxity from 0° to 90° of knee flexion.

Who Is Considered a Potential Victim, Perpetrator, or Bystander? A Systematic Review and Meta-Analysis of Research Evaluating Gender-Specific Campus Sexual Assault Prevention Programs Implemented in the United States.

Campus sexual assault is a problem that overwhelmingly affects cisgender women and transgender, genderqueer/questioning, and nonbinary (TGQN) students. Yet, students of any gender may be perpetrators or victims of assault. Thus, it is important that prevention programs incorporate a range of scenarios that depict different genders as both perpetrators and victims, while also acknowledging the differential risk between gender groups.

Multiligament knee injury (MLKI): an expert consensus statement on nomenclature, diagnosis, treatment and rehabilitation.

Multiligament knee injuries (MLKIs) represent a broad spectrum of pathology with potentially devastating consequences. Currently, disagreement in the terminology, diagnosis and treatment of these injuries limits clinical care and research. This study aimed to develop consensus on the nomenclature, diagnosis, treatment and rehabilitation strategies for patients with MLKI, while identifying important research priorities for further study.

Halving of the meniscectomy rate and their costs in Italy: A 15-years period analysis.

Purpose: The purpose of this study was to determine the incidence and hospitalization trends of meniscectomy in Italy from 2001 to 2016. A secondary aim was to investigate the economic burden of the disease on the national healthcare system.

Methods: Data were extracted from the Italian Ministry of Health's National Hospital Discharge Reports.

ACL Injury Prevention Education Improves Implementation of Neuromuscular Training Among High School Sports Coaches: A Cross-Sectional Survey Study.

Background: Neuromuscular training (NMT) programs delivered by trained personnel have demonstrated protective effects against anterior cruciate ligament (ACL) injury among high school sports participants, but few studies have investigated the impact of education on high school sports coaches' knowledge and incorporation of NMT programs into daily practice sessions.

Purpose: We sought to evaluate changes in knowledge and behavior among high school sports coaches who completed an NMT-based injury prevention training program.

Methods: High school sports coaches were invited to complete a free online training course in incorporating NMT into daily practice sessions.

Gender-Sexuality Alliance Experiences and LGBTQ+ Inclusive School Policies and Practices Predict Youth's School Belonging.

Gender-Sexuality Alliances (GSAs) and inclusive school policies and practices that affirm youth with minoritized sexual orientations or gender identities (e.g., lesbian, gay, bisexual, transgender, queer youth; LGBTQ+ youth) are two sources of support for LGBTQ+ youth that could promote school belonging.

The Predictors of Surgery for Symptomatic, Atraumatic Full-Thickness Rotator Cuff Tears Change Over Time: Ten-Year Outcomes of the MOON Shoulder Prospective Cohort.

Background: A prospective cohort study was conducted to assess the predictors of failure of nonoperative treatment, defined as the patient undergoing surgery for symptomatic, atraumatic full-thickness rotator cuff tears. We present the 10-year follow-up data of this population to determine if predictors for surgery change over time, and secondarily we report the outcomes of the cohort.

Methods: At the time of enrollment, demographic, symptom, rotator cuff anatomy, and patient-reported outcome data were collected in patients with symptomatic, atraumatic full-thickness rotator cuff tears.

How Gender-Sexuality Alliances cultivate a sense of school belonging from week to week for LGBTQ+ youth.

School belonging can facilitate positive youth development and educational outcomes. Given that LGBTQ+ youth face marginalization in schools, there is a need to identify school supports that could still promote their sense of school belonging. We considered Gender-Sexuality Alliances (GSAs) as LGBTQ+ affirming school clubs.

Meniscus Tears in Elite Athletes: Treatment Considerations, Clinical Outcomes, and Return to Play.

Purpose Of Review: Management of meniscal injuries in the elite athlete is a difficult problem secondary to the high demands of athletic competition, the need for a timely return to sport, and the desire to maximize performance over time. The purpose of this review is to provide an up-to-date summary on the current literature and trends regarding the management of meniscus injuries with a special consideration for elite athletes.

Recent Findings: Historically, partial meniscectomy has been the primary treatment option for meniscus injuries.

Anxiety, depression, somatization and psychological distress before and 2-6 years after a late termination of pregnancy due to fetal anomalies.

Background: For many women, a late termination of pregnancy (TOP) can be an enormous psychological burden. Few studies have investigated the long-term psychological impact of late TOP.

Methods: N = 90 women answered a questionnaire containing questions about anxiety, depression and somatization (Brief-Symptom Inventory, BSI-18) shortly before (T1) and 2-6 years after (T4) their late termination of pregnancy.

Bacterial coinfections in COVID-19-hospitalized patients.

Objective: The aim of this study was to assess the rate of bacterial infections in COVID-19-hospitalized patients and to analyze the most prevalent germs, sources, risk factors, and its impact on in-hospital mortality.

Methods: This observational retrospective study was conducted on 672 patients hospitalized between April and August 2020 in Nossa Senhora da Conceição Hospital, a public hospital located in Porto Alegre, Brazil. The inclusion criterion was adult patients hospitalized with confirmed COVID-19.
