Lymph node metrics following neoadjuvant therapy to refine patient selection for adjuvant chemotherapy in resected pancreatic cancer: A multi-institutional analysis.

Background: In patients with localized pancreatic ductal adenocarcinoma (PDAC) undergoing neoadjuvant therapy (NAT) and resection, selection of adjuvant chemotherapy (AC) is typically guided by high-risk features on histopathologic examination. We evaluated the interaction between post-NAT lymph node metrics and AC receipt on survival.

Methods: Patients who received NAT followed by pancreatectomy (2010-2020) at seven centers were reviewed.

Recent advances in the treatment of childhood cancers.

Purpose Of Review: Although cancer remains the leading nonaccidental cause of mortality in children, substantial advances in care have led to 5-year overall survival exceeding 85%. However, improvements in outcomes have not been uniform across malignancies or strata of social determinants of health. The current review highlights recent areas of advancement and anticipated directions for future progress.

Nickel-Imidazolium Low Transition Temperature Mixtures with Lewis-Acidic Character.

Low transition temperature mixtures (LTTMs) are a new generation of solvents that have found extensive application in organic synthesis. The interactions between the components often generate highly activated, catalytically active species, thus opening the possibility of using LTTMs as catalysts, rather than solvents. In this work, we introduce a nickel-based imidazolium LTTM, study its thermal behavior and explore its catalytic activity in the solvent-free allylation of heterocycles with allylic alcohols.

Photochemical Access to Substituted β-Lactams and β-Lactones via the Zimmerman-O'Connell-Griffin Rearrangement.

A photomediated protocol giving facile access to substituted β-lactam and β-lactones is presented. The method realizes, for the first time, the use of the Zimmerman-O'Connell-Griffin (ZOG) rearrangement in [2 + 2] cycloaddition. Products are obtained in high yield with excellent diastereoselectivity under visible light irradiation and under mild reaction conditions.

Mycelium-bound chlorogenate hydrolase of Aspergillus niger AKU 3302 as a stable immobilized biocatalyst.

CHase catalyzes chlorogenic acid (CGA) hydrolysis to yield equimolar quinic (QA) and caffeic (CA) acids, products of high value and keen industrial interest. We proposed the preparation and characterization of the nonviable mycelium of Aspergillus niger AKU 3302 containing a cell-associated CHase (CHase biocatalyst) for application in hydrolyzing the CGA from yerba mate residues to produce QA and CA. When the vegetative mycelium was heated at 55 °C for 30 min, no loss of CHase activity occurred, but vegetative mycelial growth and spore germination ended.

Ionic Organic Solid 1,3-Bis(sulfomethyl)imidazoliumate as an Effective Metal-Free Catalyst for Sustainable Organic Syntheses.

The 1,3-bis(sulfomethyl)imidazole (bsmim) was effectively prepared by a multicomponent reaction, employing aminomethanesulfonic acid, glyoxal, and formaldehyde. The catalytic activity of bsmim was tested in the synthesis of quinoline derivatives, by means of the Friedländer reaction, and in the allylic substitution of alcohols with nitrogen-containing heterocycles. The performance of sulfo-imidazole derivative (bsmim) resulted in higher comparison with the carboxyimidazole analogs (bcmim and bcmimCl), under the same reaction conditions.

Minimally invasive surgery for pediatric dumbbell neuroblastoma: systematic literature review and report of a single-stage neurosurgical and thoracoscopic approach.

Introduction: Traditional surgical strategies for dumbbell neuroblastoma entail, among others, high risk of spinal deformity. Less invasive procedures might reduce these sequelae, however, there is small evidence comparing different strategies. Indications of minimally invasive surgery in neuroblastoma are still developing.

Impact of perioperative factors on nadir serum prostate-specific antigen levels after holmium laser enucleation of prostate.

Objective: To investigate the relationship of preoperative prostate size, urinary retention, positive urine culture, and histopathological evidence of prostatitis or incidental prostate cancer on baseline and 3-month nadir prostate-specific antigen (PSA) value after Holmium laser enucleation of prostate (HoLEP).

Patients And Methods: Data from 90 patients who underwent a HoLEP by En-bloc technique were analyzed. PSA values at baseline and at 3-month follow-up, preoperative urinary retention and urine culture status, weight of resected tissue, and histopathological evidence of prostatitis or prostate cancer were recorded.

A new role for circulating T follicular helper cells in humoral response to anti-PD-1 therapy.

Background: Lung cancer is one of the most frequent malignancies in humans and is a major cause of death. A number of therapies aimed at reinforcing antitumor immune response, including antiprogrammed cell death protein 1 (anti-PD-1) antibodies, are successfully used to treat several neoplasias as non-small cell lung cancer (NSCLC). However, host immune mechanisms that participate in response to anti-PD-1 therapy are not completely understood.

CA19-9 capability as predictor of pancreatic cancer resectability in a Spanish cohort.

CA19-9 serum has been suggested as a marker of unresectability but different cut-off levels have been published. A cut-off of 500 U/ml is currently considered in an international consensus as biological criteria of borderline resectable pancreatic adenocarcinoma. To evaluate whether serum CA19-9 threshold of 500 U/ml could be adequate predictor of resectability in pancreatic adenocarcinoma.

Antimicrobial Susceptibility-Guided Therapy Versus Empirical Concomitant Therapy for Eradication of Helicobacter pylori in a Region with High Rate of Clarithromycin Resistance.

Background: Nonbismuth quadruple (concomitant) regimen is recommended for first-line empirical Helicobacter pylori (HP) eradication treatment when clarithromycin resistance is more than 15-20%. Our objective was to evaluate the efficacy and tolerability of concomitant versus antimicrobial susceptibility-guided treatment in an area with high rates of clarithromycin resistance.

Methods: Three hundred consecutive HP-infected patients received antimicrobial susceptibility-guided therapy or empirical concomitant therapy for 10 days.

Clarithromycin for first-line treatment of Helicobacter pylori infection after culture in high-resistance regions.

Objective: Resistance to antibiotics is the major cause of treatment failure of Helicobacter pylori (HP) infection. The culture-guided triple therapy (chosen on the basis of a preliminary in-vitro susceptibility test) might help to increase treatment success in high antibiotic resistance regions. The aim of this study was to evaluate the effectiveness of treatment with clarithromycin in patients with clarithromycin-sensitive culture compared with patients treated empirically.
