Background: Given the public release of large language models, research is needed to explore whether older adults would be receptive to personalized medication advice given by artificial intelligence (AI) tools.

Objective: This study aims to identify predictors of the likelihood of older adults stopping a medication and the influence of the source of the information.

Methods: We conducted a web-based experimental survey in which US participants aged ≥65 years were asked to report their likelihood of stopping a medication based on the source of information using a 6-point Likert scale (scale anchors: 1=not at all likely; 6=extremely likely).

Article Synopsis
  • The study investigates how HIV infection within a household affects the shedding of Kaposi’s sarcoma-associated herpesvirus (KSHV) among individuals.
  • Out of 469 enrolled individuals from 90 households, 44% were found to be KSHV shedders, with a notable link between the number of KSHV-positive household members and the likelihood of shedding.
  • Findings indicate that younger individuals (ages 10-19) are more likely to consistently shed KSHV and have higher viral loads, especially in households with people living with HIV.

Article Synopsis
  • The Cancer Immunotherapy Trials Network 12 study showed that pembrolizumab is safe and effective for treating advanced Kaposi sarcoma (KS) in people with HIV on antiretroviral therapy.
  • In a phase I trial involving 32 participants, 62.1% had a positive response to pembrolizumab, with even higher rates (87.5%) for those who hadn't received previous KS treatments.
  • The treatment led to significant progression-free survival, with a median of 28.2 months, and immune-mediated adverse events were effectively managed according to established guidelines.

Article Synopsis
  • The study analyzed how HIV infection in households affects the shedding of Kaposi's sarcoma-associated herpesvirus (KSHV) among participants, involving 469 individuals from 90 households sampled over three months.
  • Out of 340 KSHV seropositive individuals, 44% were found to be ever shedders, with a notable association between the number of KSHV-positive household members and the likelihood of shedding.
  • The findings revealed that younger individuals (ages 10-19) were more often consistent shedders, and both higher viral loads and shedding rates were linked to increased household members and the presence of individuals living with HIV.

Habitat fragmentation can negatively impact wildlife populations by simplification of ecological interactions, but little is known about how these impacts extend to host-associated symbiotic communities. The symbiotic communities of amphibians play important roles in anti-pathogen defences, particularly against the amphibian chytrid fungus (). In this study, we analyse the role of macroparasitic helminth communities in concert with microbial communities in defending the host against infection within the context of forest fragmentation.

Recently published near full-length KSHV genomes from a Cameroon Kaposi sarcoma case-control study showed strong evidence of viral recombination and mixed infections, but no sequence variations associated with disease. Using the same methodology, an additional 102 KSHV genomes from 76 individuals with KSHV-associated diseases have been sequenced. Diagnoses comprise all KSHV-associated diseases (KAD): Kaposi sarcoma (KS), primary effusion lymphoma (PEL), KSHV-associated large cell lymphoma (KSHV-LCL), a type of multicentric Castleman disease (KSHV-MCD), and KSHV inflammatory cytokine syndrome (KICS).

As the number of patients with congenital heart disease (CHD) continues to increase, the burden of heart failure (HF) in this population requires innovative strategies to individualize management. Given the success of implanted invasive hemodynamic monitoring (IHM) with the CardioMEMS HF system in adults with acquired HF, this is often suggested for use in patients with CHD, though published data are limited to case reports and case series. Therefore, this review summarizes the available published reports on the use of IHM in patients with complex CHD, describes novel applications, and highlights future directions for study.

Objective: Kaposi sarcoma is a vascular tumor that affects the pulmonary system. However, the diagnosis of airway lesions suggestive of pulmonary Kaposi sarcoma (pKS) is reliant on bronchoscopic visualization. We evaluated the role of Kaposi sarcoma herpesvirus (KSHV) viral load in bronchoalveolar lavage (BAL) as a diagnostic biomarker in patients with bronchoscopic evidence of pKS and evaluated inflammatory cytokine profiles in BAL and blood samples.

Surgical intervention is often used in the management of heart failure in patients with adult congenital heart disease. This review addresses anatomic variations and complications due to prior surgical interventions, including sternal reentry, collateral vessels, and the neo-aortic root after the Damus-Kaye-Stansel procedure. Surgical considerations for systemic atrioventricular valvular surgery, Fontan revision, and advanced heart failure therapies including ventricular assist devices, heart transplant, and combined heart-liver transplant are discussed, with a focus on unique patient populations including those with systemic right ventricles and those with Fontan circulation.

Health disparities are driven by unequal conditions in the environments in which people are born, live, learn, work, play, worship, and age, commonly termed the Social Determinants of Health (SDoH). The availability of recommended measurement protocols for SDoH will enable investigators to consistently collect data for SDoH constructs. The PhenX (consensus measures for Phenotypes and eXposures) Toolkit is a web-based catalog of recommended measurement protocols for use in research studies with human participants.
