Article Synopsis
  • The study investigates hyperthermic intravesical chemotherapy (HIVEC) as a bladder-sparing treatment for patients with Bacillus Calmette-Guérin (BCG)-unresponsive non-muscle invasive bladder cancer (NMIBC) who may refuse or be unfit for radical cystectomy.
  • Data from 56 patients treated with HIVEC was analyzed, focusing on high-grade recurrence-free survival (HG-RFS), complete response rates, and adverse events over a median follow-up of 32.2 months.
  • Results showed a 2-year HG-RFS of 35% and a 70% complete response rate for patients with carcinoma in situ, indicating HIVEC as a viable alternative with manageable side effects.

Background: Adjuvant intravesical instillations with (BCG) is the recommended treatment option for patients with intermediate-and high-risk non-muscle invasive bladder cancer (NMIBC). Despite adequate BCG treatment, a large proportion of patients experience a recurrence. Although radical cystectomy is the gold standard for BCG unresponsive NMIBC, some patients are unfit or unwilling to consider this option.
