* The study aimed to explore whether DBS affects contextual anxiety, using a double-blind design with patients experiencing active vs. sham stimulation.
* Results showed that while DBS improved overall anxiety symptoms, it did not influence the startle response linked to contextual anxiety, suggesting OCD-related anxiety may involve different brain circuits than those seen in other anxiety disorders.

Obsessive-compulsive disorder (OCD) is a chronic psychiatric disorder characterized by persistent thoughts and repetitive ritualistic behaviours. Despite optimal cognitive-behavioral and pharmacological therapy, approximately 10 % of patients remain treatment-resistant. Deep brain stimulation (DBS) is being investigated as experimental therapy for treatment-refractory OCD.
