Objective: Evaluate sex differences in patients undergoing repair of acute type A aortic dissection (ATAAD).

Methods: Sex-stratified, single-center cohort study of patients undergoing ATAAD repair from 1997 to 2022. The primary outcome was aortic diameter at time of presentation with ATAAD.

The saphenous vein graft (SVG) has been a cornerstone of coronary bypass surgery, but its long-term patency is limited by accelerated atherosclerosis. Recent advancements, including the no-touch technique and the use of SVG as a limb of the left internal thoracic artery (LITA), have shown promise in improving outcomes. Both approaches enhance nitric oxide (NO) availability, a key factor in promoting endothelial stability and arterial-like behavior in the SVG.

Importance: It remains unknown whether outcomes of the Placement of Aortic Transcatheter Valves 3 (PARTNER 3) and Evolut Low Risk trials are comparable with surgical outcomes in nontrial settings, considering the added risk of concomitant cardiac operations.

Objective: To compare 30-day mortality and stroke incidences of patients in the surgical aortic valve replacement (SAVR) arm of low-risk trials with those of similar patients in the US Society of Thoracic Surgeons Adult Cardiac Surgery Database (STS ACSD).

Design, Setting, And Participants: A cross-sectional sampling study was conducted of adults in the STS ACSD with severe aortic stenosis at low surgical risk for AVR who underwent SAVR during the years low-risk AVR trials (PARTNER 3 and Evolut Low Risk) were enrolling (calendar years 2016-2018).

Endothelial cells control the vascular tone of arterial grafts used for coronary artery bypass surgery and react to changes in local shear stress. The vascular adaptations induced by endothelial cell activation affect the outcome of surgical grafts and can be predicted based on the set-point theory. In this Hypothesis piece, it is proposed that the set-point concept should inform surgical decision making for coronary artery bypass surgery.

Background: In the PROTECTION trial, intravenous amino acids (AA) decreased the occurrence of acute kidney injury (AKI) in cardiac surgery patients with cardiopulmonary bypass (CPB). Recruitment of renal functional reserve may be responsible for such protection. However, patients with chronic kidney disease (CKD) have diminished renal functional reserve, and AA may be less protective in such patients.

The structural and functional integrity of conduits used for coronary artery bypass grafting is critical for graft patency. Disruption of endothelial integrity and endothelial dysfunction are incurred during conduit harvesting subsequent to mechanical or thermal injury and during conduit storage prior to grafting, leading to acute thrombosis and early graft failure. Late graft failure, in particular that of vein grafts, is precipitated by progressive atherogenesis.

Background: There is concern that left internal thoracic artery (LITA)-to diagonal (D)-to left anterior descending artery (LAD) grafts may be more susceptible to failure compared with single LITA-LAD grafts.

Methods: Pooled individual patient data from 8 clinical trials with systematic graft imaging were analyzed to assess the incidence of sequential LITA-D-LAD vs single LITA-LAD grafts. Mixed-effects multivariable logistic regression, adjusting for patient characteristics and clustering within trials, was used.

Background: The role of sex in choosing between coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) for unprotected left main coronary artery (ULMCA) disease has gained interest.

Methods: Randomized controlled trials and adjusted observational studies comparing PCI versus CABG in ULMCA patients with outcomes by sex were included. The primary endpoint was major adverse cardiovascular events (MACE), with secondary endpoints being all-cause mortality and repeated revascularization.

Subclavian artery aneurysms are rare and can result in thromboembolism or rupture. We present the case of a 41-year-old man with a history of Marfan's syndrome and multiple previous operations, who presented with an enlarging asymptomatic 5.2 cm right subclavian artery aneurysm and was successfully treated with a hybrid surgical operation.

Article Synopsis
  • - The integrity of conduits for coronary artery bypass grafting is crucial for their long-term success, as damage during harvesting and storage can lead to graft failure or thrombosis.
  • - Effective intra-operative management involves choosing the right techniques for harvesting and protecting the conduits, as well as using vasodilators to prevent vasospasm.
  • - Post-operative care includes continuing vasodilator therapy and using medications to prevent atherosclerosis progression, with the review highlighting both established practices and areas needing further study.

Article Synopsis
  • Acute kidney injury (AKI) is a frequent complication that can occur during surgery, and currently, there's no proven single method for its prevention; intravenous amino acids (AA) might help protect kidney function during this period.
  • A meta-analysis of 15 studies (involving over 4,500 patients) showed that patients receiving AA had a lower incidence of AKI (24.7%) compared to those who didn't (30.1%), indicating a significant potential benefit of AA infusion.
  • The findings also revealed that AA administration not only lowered the risk of AKI but also reduced serum creatinine levels and hospital stays while improving kidney function.

Article Synopsis
  • The study investigates the best revascularization method for diabetic patients with multivessel disease who experience a non-ST-segment elevation myocardial infarction (NSTEMI), comparing coronary artery bypass grafting (CABG) and multivessel percutaneous coronary intervention (PCI).
  • It involves a large cohort of over 11,000 patients and reveals that CABG is linked to lower all-cause mortality than PCI, particularly in patients deemed potentially ineligible for CABG after a surgical consultation.
  • The results suggest that while CABG is generally more beneficial for these patients, the advantage is more pronounced when comparing CABG to a specific group of PCI patients who had a surgical consultation beforehand.

Article Synopsis
  • The review highlights a lack of evidence in coronary revascularization decisions specifically for women and minorities and introduces the RECHARGE trial program aimed at addressing this gap.
  • The RECHARGE program includes two trials: one focused on women and the other on Black or Hispanic patients, comparing CABG and PCI treatments over a follow-up period of up to 10 years.
  • Key outcomes will assess quality of life and mortality, with a focus on patient-centered measures, making these trials unique in their approach to these underrepresented populations.

Objective: Reoperative aortic root replacement (ARR) is a technically challenging procedure. This study assesses the influence of reoperation on outcomes following ARR, particularly after prior acute type A aortic dissection repair.

Methods: Of the 1823 patients in this study, 1592 (87.

Background: Recent evidence has suggested use of the right internal mammary artery (RIMA) is associated with worse clinical outcomes in patients undergoing coronary artery bypass grafting (CABG) surgery. Therefore, we compared the clinical consequences of secondary conduit selection (RIMA vs radial artery vs saphenous vein) after CABG.

Methods: A post-hoc analysis of the CABG Off or On Pump Revascularization Study, involving 3913 patients from 79 centers in 19 countries who underwent CABG surgery and received arterial grafting with at least 2 grafts.

Objectives: Clinical trials that are terminated prematurely may generate incomplete and potentially biased data and the reasons for premature trials termination are poorly understood. Our objective was to describe the incidence of premature trial termination and identify factors associated with it.

Methods: We performed a systematic search on ClinicalTrials.

Objective: We sought to evaluate outcomes of valve-sparing root replacement (VSRR) in patients with bicuspid aortopathy (BAV) versus other connective tissue disorder (CTD).

Methods: This was a single-center cohort study of consecutive patients undergoing VSRR via reimplantation from 2000 to 2023 with BAV or CTD. Operative outcomes, Kaplan-Meier survival estimates, and cumulative risk of reoperation and recurrent aortic insufficiency (AI) with the competing risk of death were assessed.
