Article Synopsis
  • Interferon gamma release assay (IGRA) is a test used to detect latent tuberculosis (TB) before starting certain biological treatments for inflammatory rheumatism.
  • A case study of a 50-year-old woman highlights that she had a negative IGRA test before starting adalimumab for her axial spondyloarthritis, but later developed disseminated TB with severe symptoms.
  • The case emphasizes the limitations of IGRA in detecting active TB, the need for thorough risk assessment before biological treatments, and the possibility of false-negative IGRA results.

Article Synopsis
  • A study conducted across 12 hospitals in France found that 26.9% of Proteus mirabilis bacteria resistant to amoxicillin-clavulanate produced the enzyme OXA-23.
  • Researchers identified that a measurement of less than 11 mm in the inhibition zone around an amoxicillin-clavulanate disc effectively indicated the presence of OXA-23-producing bacteria.
  • Whole-genome sequencing showed that all OXA-23 producers belonged to the same lineage known for spreading OXA-23 or OXA-58 in P. mirabilis.

Molecular diagnosis on nasopharyngeal swabs (NPS) is the current standard for COVID-19 diagnosis, but saliva may be an alternative specimen to facilitate access to diagnosis. We compared analytic performances, feasibility and acceptability of NPS, saliva, and oral-self sampling swab for the detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). A prospective, multicenter study was conducted in military hospitals in France among adult outpatients attending COVID-19 diagnosis centers or hospitalized patients.

The Alere-i™ Influenza A&B (Abbott), a nicking endonuclease amplification reaction test, has recently been improved in order to deliver results in a few minutes. Our field observation highlights two problems with this new version: improper interpretation of a test as valid despite improper reagent hydration and falsely influenza B positive results. We advise users of the new system to check reagent hydration prior to reporting a result and to systematically confirm positive influenza B results.

Article Synopsis
  • The study aimed to explore how sub-MICs of antistaphylococcal antibiotics affect virulence factor expression in CA-MRSA strains.
  • Clindamycin and linezolid were found to significantly lower the mRNA levels of Panton-Valentine leucocidin (PVL) and protein A (SpA), while tigecycline had a moderate effect, and daptomycin and vancomycin showed minimal impact.
  • Overall, antibiotic type influenced the suppression of virulence factors, with clindamycin demonstrating the strongest and most consistent inhibitory effects across various strains.

Amebiasis, the parasitic disease caused by Entamoeba histolytica, may result in extra-intestinal diseases among which liver abscess is the most common manifestation. We report two cases of amebic liver abscess illustrating the inequal sensitivity of serologic tests detecting anti-amebic antibodies.

Streptococcus pneumoniae has been rarely considered as an infectious agent in appendicitis. We report a case of a 47-year-old woman with acute appendicitis caused both by serotype 35B S. pneumoniae and Klebsiella pneumoniae.

We report the case of an asymptomatic patient presenting a severe chronic renal hypokalaemia. Once being sure of no diuretics use, two hypothesis can be mentioned for a normotensive patient presenting an hypokalaemia associated with a metabolic alcalosis: Bartter syndrome or Gitelman syndrome. The highlighting of low magnesaemia and hypocalciuria strongly concentrates the diagnosis on Gitelman syndrome.
