Importance: Recent evolutions in clinical care and remote monitoring suggest that some acute illnesses no longer require intravenous therapy and inpatient hospitalization.

Objective: To describe outcomes of patients receiving care in a new, outpatient, virtual, home-based acute care model called Safer@Home.

Design, Setting, And Participants: This retrospective cohort analysis, conducted from September 1, 2022, through August 31, 2023, included 2466 patients treated at a safety net hospital in Los Angeles County for 10 core illnesses and 24 other acute illnesses for which patients are commonly hospitalized.

Various genetic elements, including integrons, are known to contribute to the development of antimicrobial resistance. Class 1 integrons have been identified in isolates and are associated with multidrug resistance in countries of the Andean Community. However, detailed information on the gene cassettes located on the variable regions of integrons is lacking.

Article Synopsis
  • * The study analyzed 40 unique isolates from clinical samples between 2018 and 2019, identifying bacteria using the API 20A system and measuring antibiotic effectiveness with Sensititre anaerobe MIC plates.
  • * Findings revealed 11 isolates resistant to metronidazole, with only 2 carrying the significant resistance gene, and 5 imipenem-resistant isolates, suggesting a need for ongoing monitoring and understanding of resistance mechanisms to improve treatment options.

The implementation of government regulations on antibiotic use, along with the public's concern for drug resistance, has strengthened interest in developing alternatives not only aimed at preserving animal production but also at reducing the effects of pathogenic infections. Probiotics, in particular, are considered microorganisms that induce health benefits in the host after consumption of adequate amounts; they have been established as a potential strategy for improving growth, especially by stimulating intestinal homeostasis. Probiotics are commonly associated with lactic acid bacteria, and is a well-studied species recognized for its favorable characteristics, including adhesion to epithelial cells, production of antimicrobial compounds, and activation of receptors that prompt the transcription of immune-associated genes.

The microspore can follow two different developmental pathways. In vivo microspores follow the gametophytic program to produce pollen grains. In vitro, isolated microspores can be reprogrammed by stress treatments and follow the embryogenic program, producing doubled-haploid embryos.

Stress-induced microspore embryogenesis is a model system of cell reprogramming, totipotency acquisition, and embryo development. After induction, responsive microspores abandon their developmental program to follow an embryogenic pathway, leading to embryo formation. This process is widely used to produce doubled-haploid lines, essential players to create new materials in modern breeding programs, particularly in cereals, although its efficiency is still low in many crop species, because the regulating mechanisms are still elusive.

Article Synopsis
  • * Out of 929 blood cultures that showed Gram-negative bacteria, 181 tested positive for E. coli, with 31.5% identified as ESBL-Ec, primarily originating from urinary and biliary tract infections.
  • * The dominant strain (ST131) of ESBL-Ec was highly virulent and adaptable, raising concerns about its role in spreading antibiotic resistance and posing a public health risk.

Omega-3 polyunsaturated fatty acids (ω-3 PUFAs) are relevant to fetal and infant growth and development. Objective: to assess whether long-term exposure to dietary ω-3 PUFA imbalance alters pre- and/or postnatal pups' development and reproductive function later in life. Mice dams were fed with ω-3 PUFA Control (soybean oil, 7%), Deficient (sunflower oil, 7%) or Excess (blend oil; 4.

Antimicrobial resistance to acnes has become a worldwide problem in the last century, but there are no previous studies on antibiotic susceptibility patterns of this bacterium in Ecuador. A total of 129 skin swabs were collected from patients with acne vulgaris (AV) attending the dermatology department of a hospital in Quito, Ecuador, from July to August 2015. The patients selected had received registered antimicrobial therapy on at least one occasion before sampling.

Purpose: In the setting of female patients in the ED receiving both a CT scan and ultrasound of the pelvis, we compared the studies to assess the diagnostic value of non-visualization of the ovary in predicting the absence of ovarian disease.

Methods: A retrospective review of the electronic medical record identified adult female non-pregnant patients who received both a CT abdomen and pelvis and a transabdominal and transvaginal pelvic ultrasound within a 48-h period through the emergency department. The imaging studies were blindly reviewed to evaluate the ovaries on each CT and US independently.

Article Synopsis
  • The study explored factors influencing 30-day mortality in cancer patients with pulmonary embolism, analyzing both suspected (SPE) and unsuspected (UPE) cases.
  • The cohort consisted of 1,033 patients, with key predictors of mortality identified, such as VTE history, cancer type, disease progression, and vital signs.
  • Mortality rates differed; truly asymptomatic UPE patients had a lower rate (3%) compared to UPE with symptoms (20%) and SPE (21%), indicating disease severity significantly impacts short-term mortality outcomes.

We report on a 72 year-old pseudophakic patient who had been implanted with a monofocal intraocular lens, and who underwent a unilateral Supracor laser-assisted in-situ keratomileusis (LASIK) procedure to correct presbyopia and hyperopia. Uncorrected near visual acuity was improved, but the patient complained of glare, halo, reduced distance vision, and poor night driving vision due to treatment decentration. One year following the surgery, a reversal procedure was conducted with a wavefront-guided aspheric treatment to reverse the presbyopic correction while still maintaining the hyperopic correction.

There have been several case reports documenting acute appendicitis complicated by perforation presenting with retroperitoneal abscess formation. To date, there are no case reports of acute appendicitis in which the only sign for retroperitoneal perforation is the presence of retroperitoneal air as detected by computed tomography (CT). In the case presented, an 18-year-old male presented to the emergency department with clinical symptoms of acute appendicitis.

Background: Cervical cancer is the third cause of cancer death among Chilean women, affecting mainly women from low socioeconomic status.

Aim: To determine main risk factors (RF) including human papiloma virus (HPV) types associated with abnormal cervical cytology (Atypical Squamous Cells of Undetermined Significance or ASCUS) among Chilean women from low socioeconomic status in Santiago, Chile.

Material And Methods: A random population based sample of 616 women from La Pintana (a low-income district in Santiago) participated in 2001 in a HPV prevalence study and were re-evaluated in 2006 through a risk factors questionnaire, Papanicolaou test and DNA detection for HPV.
