This work aims at investigating the supply and demand of the medium-complexity services from the Federal District and surrounding municipalities from the state of Goiás. Our hypothesis is that the services organization offer of primary care (PC) from the Federal District (DF) and its surroundings overloads the medium-complexity services (MC), contributing to less equity in terms of communities access. Our objective is to analyze the demand for MC (medium-complexity) services in DF (Federal District) and its relation with PC (Primary Care); to quantify the origin, health problem, reason, transportation time and performed procedures of the ones who go to the Federal District hospitals; to set up priorities of managerial changes in the Brazilian Health System (SUS) in order to have more access to users.

This article analyzes the history of primary health care policy in the Federal District, Brazil, based on the theoretical framework of historical neo-institutionalism, identifying the predominant configurations and trends in the various administrations of the State Health Secretariat (SES-DF) from 1960 to 2007. The study indicates that the characteristics of the Federal District's health policy are dependent on the history of the original health system plans for setting priorities and goals, as well as for the health system's implementation. This influence, in addition to the centralization of decision-making processes and limited political participation, can contribute to making primary care ancillary to hospital care, thus jeopardizing its potential to produce change in the health care model.

This theoretical study focuses on the influence of myths and historical traditions of Nursing in professional practice, and is based on the emancipatory trihedron of care: to know in order to better care, to care in order to confront, to care in order to emancipate. The analysis of nurses' political fragility unveiled conservative and positivist tendencies that have been historically associated with the profession, leading to a theoretical reflection on conservative discourses about Nursing. Through the emancipatory care perspective, the study aims at pointing out possibilities for the reconstruction of historical practices of health care, adding dimension to social practices.

Such notion can mainly be expressed by the trihedron getting to know to care better, delivering care to confront, delivering care to emancipate which, when adapted to the work process in health, may become emancipatory. This theoretical-philosophical reflection explores the ambiguous aspect of care, expressed by being solidary and becoming political.
