Impact of adenotonsillectomy and palatal expansion on the apnea-hypopnea index and minimum oxygen saturation in nonobese pediatric obstructive sleep apnea with balanced maxillomandibular relationship: A cross-over randomized controlled trial.

Objective: To determine the impact and best management sequence between adenotonsillectomy (AT) and rapid palatal expansion (RPE) on the apnea-hypopnea index (AHI) and minimum oxygen saturation (MinSaO) in nonobese pediatric obstructive sleep apnea (OSA) patients presenting balanced maxillomandibular relationship.
Study Design/methods: Thirty-two nonobese children with balanced maxillomandibular relationship and a mean age of 8.8 years, with a graded III/IV tonsillar hypertrophy and maxillary constriction, participated in a cross-over randomized controlled trial.

- The study aimed to evaluate the impact of adenotonsillectomy (AT) and rapid maxillary expansion (RME) on the apnea-hypopnea index (AHI) in children, as well as the volumetric changes in their upper airway after each treatment.
- Thirty-nine children with maxillary constriction and enlarged tonsils were divided into two groups to receive either AT or RME, with assessments of AHI and airway volume conducted before and after the treatment.
- Results indicated that AT significantly improved AHI more than RME, largely due to greater increases in upper airway volume, highlighting AT's effectiveness in treating pediatric obstructive sleep apnea.

Melatonin has been shown to improve sleep in several conditions. In experimental models of PD, melatonin can ameliorate motor symptoms.
