[The integration between hospitals and territorial services in order to grant continuity in Health Care].

This study focuses on the interaction between hospitals and territorial services, namely the interaction between health services and activities aimed at providing continuous health care to the population. It is one major subject of the Health Reform, whose implementation has been in progress for more than thirty years and is described by the authors as very controversial.
