Injuries of the peroneal tendon complex are common and should be considered in every patient who presents with chronic lateral ankle pain. These injuries occur as a result of trauma (including ankle sprains), in tendons with preexisting tendonopathy, and with repetitive microtrauma due to instability. The peroneus brevis and peroneus longus tendons are rarely torn simultaneously.

This study investigated the use of a custom cavus foot orthosis (CFO) in the treatment of ankle instability and pain associated with the subtle cavus foot, a common pathological foot alignment in the United States population. Patients referred by a single orthopedic foot and ankle surgeon to a single pedorthotist for a CFO over a 2-year period were eligible. Pain score pre-and postorthosis and number of instability events pre- and postorthosis were retrospectively evaluated.

The subtle cavus foot alignment is associated with many injuries occurring in the athletic population, most commonly ankle instability and lateral foot overload. Although correction of the primary injury may address the manifestations of the subtle cavus foot, it does not change the alignment. Without recognition of this alignment, the injury likely will recur.

Background: Heel cord lengthening is a common component of foot and ankle surgery. If the tendon is anatomically or functionally over lengthened patients may develop plantarflexion weakness and heel overload problems such as symptomatic plantar heel callosities and heel ulceration.

Methods: Nine patients who developed heel overload or heel ulcer after a heel cord lengthening or an irreparable rupture were identified.

Background: Gymnastics is a competitive and popular sport that is started at an early age, and elite female gymnasts reach their prime in mid-teenage years. The level of intensity of practice and competition, the number of events, and the degree of difficulty of the maneuvers make gymnastics one of the most injury-producing sports.

Methods: Over a 3-year period, 14 elite, female gymnasts were seen in one foot and ankle center.

Ninety-nine patients undergoing primary total knee arthroplasty were prospectively evaluated for pre-resurfacing and post-resurfacing patellar thickness and medial patellar liftoff at 30 degrees of flexion without manual pressure on the patella. Regardless of medial patellar liftoff, no lateral releases were performed. Tibiofemoral angles, patellar tilt, and patellar congruence angles were measured preoperatively and postoperatively.

We evaluated the S-ROM bolt and washer (DePuy, Inc, Warsaw, IN) for fixation of the greater trochanter after trochanteric osteotomy in 29 hips in 28 consecutive patients. Of 29 cases, 9 (31%) resulted in trochanteric nonunion, with bony union being achieved in only 38% of cases. There was dislocation and subluxation in 7 of 29 (24%) cases with 6 of the 7 dislocations associated with trochanteric nonunion.
