The diagnosis of inflammatory bowel disease (IBD) in children and the need to distinguish between subtypes (Crohn's disease (CD) and ulcerative colitis (UC)) requires lengthy investigative and invasive procedures. Non-invasive, rapid, and cost-effective tests to support these diagnoses are needed. Faecal volatile organic compounds (VOCs) are distinctive in IBD.

Article Synopsis
  • Eosinophilic esophagitis (EoE) is a chronic condition affecting the esophagus, characterized by inflammation and eosinophil infiltration, necessitating updates to existing guidelines due to new medical knowledge.
  • A group of pediatric gastroenterologists reviewed recent studies to create 52 statements and 44 recommendations related to EoE, focusing on diagnostics, treatment options, and diet changes from 2014 to 2022.
  • The updated guidelines highlight the emergence of biologics for treatment, the potential role of steroids for esophageal strictures, and the importance of addressing quality of life and transitioning patients to adult care.

Article Synopsis
  • This study examined the differences in fecal volatile organic compounds (VOCs) in children newly diagnosed with inflammatory bowel disease (IBD) based on disease type and severity.* -
  • Researchers found that 43.6% of the VOCs were significantly decreased in children with Crohn's disease and ulcerative colitis, particularly certain fatty acids and alcohols, indicating a unique profile related to IBD.* -
  • Despite clinical improvements with treatment, the overall number and amounts of VOCs did not increase, highlighting ongoing dysbiosis, with specific compounds showing distinct changes linked to treatment outcomes.*

Introduction: Eosinophilic gastrointestinal disorders beyond eosinophilic esophagitis (non-EoE EGIDs) are rare chronic inflammatory disorders of the gastrointestinal (GI) tract. Diagnosis is based on clinical symptoms and histologic findings of eosinophilic inflammation after exclusion of a secondary cause or systemic disease. Currently, no guidelines exist for the evaluation of non-EoE EGIDs.

Background: Therapeutic options for paediatric inflammatory bowel disease (IBD) are limited, especially for younger children. Unlike in adults, vedolizumab and ustekinumab are not licensed for paediatric use in the UK. We aimed to understand the real-world access to, and use of, these therapies in the paediatric population.

Article Synopsis
  • The pEEr registry was created to gather and analyze data on children with eosinophilic esophagitis (EoE) from various centers in Europe and Israel, focusing on phenotypic characteristics.
  • A total of 582 children were analyzed, revealing a median diagnosis age of 10.5 years and a common delay in diagnosis, especially in children under 6, with variations in symptoms based on age.
  • Older children more frequently experienced dysphagia and food impaction, while younger children showed non-specific symptoms; treatments differed, with many unresponsive patients receiving topical steroids or elimination diets.

Background: Accurate measurements of mucosal eosinophil concentrations in gastrointestinal tracts of healthy children are necessary to differentiate health and disease states in general, and better define eosinophilic gastrointestinal diseases.

Study: We retrospectively reviewed gastrointestinal biopsies from children with macroscopically normal endoscopies, who, after a minimal follow-up of one year, were not diagnosed with any organic disease. Peak eosinophil concentrations and distributions were assessed from each segment of the gastrointestinal tract.

Unlabelled: Iron deficiency is common in children and can have negative effects on behavior and function. Standard oral ferrous iron replacement is poorly absorbed and can cause treatment-limiting gastrointestinal adverse events (AEs). Ferric maltol is formulated to improve gastrointestinal absorption and tolerability versus oral ferrous compounds.

Article Synopsis
  • The study investigates the effectiveness of systemic steroids in treating esophageal strictures in children with Eosinophilic Esophagitis (EoE), aiming to clarify their role in treatment.* -
  • Twenty children with EoE and varying degrees of esophageal strictures were reviewed, with most showing significant clinical improvement after receiving systemic steroids, resulting in high rates of stricture resolution.* -
  • Minor side effects were noted, and the findings suggest that short-term systemic steroid use could reduce the need for mechanical dilation in these patients.*

Article Synopsis
  • - The study evaluated how pediatric (PG) and adult gastroenterologists (AG) in Europe and the UAE diagnose and manage eosinophilic esophagitis (EoE), focusing on their adherence to clinical guidelines.
  • - Results showed that PG are more likely to take biopsies and perform endoscopic follow-ups compared to AG; they also prefer elimination diets over steroids as a second-line therapy after proton-pump inhibitors fail.
  • - The findings highlight significant practice differences between PG and AG, as well as regional variations across countries, suggesting that educational initiatives could help improve alignment with current guidelines.
