Objective: To study risk factors of hospital admission among older Thai adults.
Material And Method: Nationwide observational cross-sectional survey by mailed questionnaires. 1801 older adults who attended senior citizen clubs organized by local hospitals from 66 provinces of Thailand.
Objective: To investigate the prevalence, occurrence and protective level of influenza infections using serology in patients with chronic obstructive pulmonary disease (COPD) during a one-year influenza vaccination study.
Material And Method: A total of 123 patients with COPD were enrolled during the period of 1997 to 1998. There were 61 patients in the vaccine group and 62 patients in the placebo group with a mean age +/- SD of 67.
Our previous studies demonstrated the high prevalence and incidence of Chronic Obstructive Pulmonary Disease (COPD) among the Bangkok elderly, as well as the cost-effectiveness of a questionnaire, chest radiography, and mini peak expiratory flow rate (miniPEF) as screening tools. This final study aimed to identify the most cost-effective screening method among individual, serial and parallel combinations of the above tools, based on the guidelines for diagnosing COPD of the Thoracic Society of Thailand. Studies of HLA antigens in 15 Thai patients with chronic obstructive pulmonary disease revealed a significant increase in HLA-Bw 60 frequency in the group with low ventilatory drive to carbondioxide using unstimulated airway pressure. The finding suggests an immunogenetic role of HLA-Bw 60 on the control of ventilation in COPD.

Multiple linear regressions thus obtained evidently suggest that airway resistance and ventilatory drive are the main factors in the control of breathing among Thai COPD subjects; the exercise tolerance test showed some correlation with airflow obstruction and with respiratory muscle strength. Clinical implementation of certain findings has been outlined.

This study was designed to investigate a group of Thai COPD patients for their respiratory centre sensitivity while breathing room-air and during CO2 rebreathing, and tests to determine exercise tolerance and degrees of respiratory dysfunction. Results in 56 patients as compared with 45 age-matched healthy subjects disclosed comparable data between both subject groups, with the single exception that one-third (35.7%) of the patients experienced stronger ventilatory drive while breathing room-air and a lower musculo-ventilation transfer index during room-air breathing as well as CO2 rebreathing.

An increase in the frequency of HLA-Bw46 and -DR4 and a decrease in the frequency of HLA-B12 were found when compared with the matched controls. Hypercapnia was induced by rebreathing, and both the ventilation and the mouth pressures produced during transient airway conclusion during inspiration were obtained. Both the chemoreceptor sensitivity to hypercapnia (delta P/delta PCO2) and the ventilatory response (delta V/delta PCO2) measured during the initial study were shown to be unrelated to prognosis when the outcome of these patients was determined four years later.

In doses of 50 mg and 100 mg given intravenously, tilidine hydrochloride induced less respiratory depression than 10 mg of morphine given intravenously. Side effects were not different or troublesome with either drug.
