Article Synopsis
  • Multiple sclerosis (MS) is an autoimmune disease that involves the immune system attacking the nervous system, and there's a key difference in gut bacteria between its progressive and non-progressive forms.
  • Researchers found a specific cluster of the gut bacteria "Tyzzerella nexilis" that is more abundant in progressive MS patients, which has unique genetic features.
  • This particular strain of T. nexilis carries mobile genetic elements that could enhance its pathogenic potential, making mice more susceptible to MS-like symptoms when exposed to it, suggesting it may play a role in disease progression.

  • The study explores the mTEIER-A surgical technique for accessing the jugular fossa (JF), considering the complex anatomy and the proximity of vital structures like the sigmoid sinus (SS).
  • Researchers conducted dissections on human head specimens, measuring SS position, horizontal angle of attack, and size using high-resolution CT scans to evaluate surgical access before and after the procedure.
  • Results indicate that certain positions of the SS and horizontal angles enhance access to the JF, suggesting that this technique may reduce the need for more invasive surgeries, highlighting the importance of careful preoperative planning.

We read with great interest the article "the deadly duo of hypertension and diabetes in India: further affirmation from a new epidemiological study" by Metri et al. They rightly pointed out that the prevalence of hypertension in Indian patients with type 2 diabetes patients is high and therefore early screening and management of hypertension should be included in the treatment of patients with type 2 diabetes. We wish to share our study findings on the prevalence of hypertension in newly onset diabetes mellitus (DM).

Background: Early secondary hyperparathyroidism (SHPT) diagnosis and treatment are crucial to delay the progression of SHPT and related complications, in particular, cardiovascular events and bone fractures. Extended-release calcifediol (ERC) has been developed for the treatment of SHPT in patients with stage 3/4 chronic kidney disease (CKD) and vitamin D insufficiency (VDI).

Summary: This review compares baseline characteristics and treatment responses of SHPT patients receiving ERC in phase 3 studies with those treated with ERC in a real-world study.

Background: Inflammation is pivotal to the progression of atherosclerosis. Cholesterol crystals (CCs) that grow and enlarge within the plaque core can cause plaque rupture and trigger inflammation as they deposit into the atherosclerotic bed. Thus, agents that affect CC formation, expansion, and morphology may reduce cardiovascular (CV) risk independent of lipid-lowering and anti-inflammatory therapy.

Article Synopsis
  • Heart failure (HF) is linked to higher risks of major bleeding and all-cause death among atrial fibrillation (AF) patients on non-vitamin K antagonist oral anticoagulants (NOACs), particularly with lower left ventricular ejection fraction (LVEF).
  • In a study of over 13,000 AF patients treated with edoxaban, 14.1% had HF, and those with HF experienced greater rates of strokes, major bleeding, and mortality compared to those without HF.
  • The findings suggest a need for improved management strategies for AF patients with HF, even though NOACs effectively reduced ischaemic event risks regardless of HF status.

Homologs of the protein Get3 have been identified in all domains yet remain to be fully characterized. In the eukaryotic cytoplasm, Get3 delivers tail-anchored (TA) integral membrane proteins, defined by a single transmembrane helix at their C terminus, to the endoplasmic reticulum. While most eukaryotes have a single Get3 gene, plants are notable for having multiple Get3 paralogs.

Background: European data pre-2019 suggest statin monotherapy is the most common approach to lipid management for preventing cardiovascular (CV) events, resulting in only one-fifth of high- and very high-risk patients achieving the 2019 ESC/EAS recommended low-density lipoprotein cholesterol (LDL-C) goals. Whether the treatment landscape has evolved, or gaps persist remains of interest.

Methods: Baseline data are presented from SANTORINI, an observational, prospective study that documents the use of lipid-lowering therapies (LLTs) in patients ≥18 years at high or very high CV risk between 2020 and 2021 across primary and secondary care settings in 14 European countries.

Background: Preterm birth or low birth weight is the single largest cause of death in newborns, however this mortality can be reduced through newborn care interventions, including Kangaroo Mother Care (KMC). Previously, a multi-country randomized controlled trial, coordinated by the World Health Organization (WHO), reported a significant survival advantage with initiation of continuous KMC immediately after birth compared with initiation of continuous KMC a few days after birth when the baby is considered clinically stable. Whether the survival advantage would lead to higher rates of neurodevelopmental morbidities, or the immediate KMC will also have a beneficial effect on cognitive development also, has not been investigated.
