Unlabelled: A 41-year-old male with prior diagnosis of seronegative myasthenia gravis presented with complaints of bilateral ptosis, dysphagia, nasal voice and nasal regurgitation. Despite treatment with pyridostigmine, there was lack of significant improvement in the symptoms. Further investigations, including antibody assays and nerve conduction studies did not support the diagnosis.

Many patients being investigated for Guillain-Barré syndrome (GBS) undergo unnecessary neuroimaging. The objective of this study was to determine the proportion of patients with GBS undergoing neuroimaging investigation, and to investigate any association with different GBS variants using the Brighton criteria. This cross-sectional observational study was conducted in the leading tertiary care hospital in Pakistan; 148 patients being investigated for, and subsequently diagnosed with GBS between January 2017 and March 2020 were enrolled.

From early 2020, a high demand for SARS-CoV-2 tests was driven by several testing indications, including asymptomatic cases, resulting in the massive roll-out of PCR assays to combat the pandemic. Considering the dynamic of viral shedding during the course of infection, the demand to report cycle threshold (Ct) values rapidly emerged. As Ct values can be affected by a number of factors, we considered that harmonization of semi-quantitative PCR results across laboratories would avoid potential divergent interpretations, particularly in the absence of clinical or serological information.

The neurological manifestations of SARS-CoV-2 are wide-ranging from simple headache to severe demyelinating brain disease. This is a review of collected case reports of patients with SARS-CoV-2 with neurological manifestations presenting to the Pakistan Institute of Medical Sciences (PIMS). Neurological manifestations associated with SARS-CoV-2 such as encephalitis, acute cerebrovascular disease, encephalitis with chorea, post-COVID myositis and Guillain-Barré Syndrome (GBS) are of great concern but are often overlooked in the presence of life-threatening abnormal vital signs in severely ill SARS-CoV-2 patients.

The sensitivity of culture in Bactec Plus Aerobic/F* culture vials of body fluids from adult patients at a university hospital was compared with that of conventional culture methods, including enrichment in Schaedler broth. Previous antibiotic therapy was recorded at the time of sampling. Analysis of culture results took account of the clinical significance of isolates and impact on therapy.
