Total body irradiation (TBI) treatments are used to treat the whole body in preparation for hematopoietic stem cell (or bone marrow) transplantation. Our standard clinical regimen is a 12 Gy in 6 fraction, bi-daily technique using 6 MV X-rays at an extended Source-to-Surface distance (SSD) of 300 cm. Utilizing these characteristics, the beam dose rate is reduced below 7 cGy/min as is standard for TBI treatment.

Cone beam computed tomography (CBCT) is now widely used to image radiotherapy patients prior to treatment for the purpose of accurate patient setup. However each CBCT image delivered to a patient increases the total radiation dose that they receive. The measurement of the dose delivered from the CBCT images is not readily performed in the clinic.

The purpose of this study was to quantify the effect of treatment area and treatment distance on dose distributions for geometrically optimised surface mould plans in order to provide guidance in choosing treatment parameters and constructing moulds for individual patients. Geometrically optimised plans were generated with a typical brachytherapy planning system and measurements were taken with radiochromic film over depths of 5-32 mm with an (192)Ir high dose rate source. Films were calibrated with a cylindrical geometry technique utilising the (192)Ir source and readout was performed with a flatbed scanner.

High dose rate intraluminal brachytherapy treatments can be delivered using as few as one or two afterloading catheters, delivering doses of up to 10 Gy at 10 mm, leading to high dose gradients and extreme hot spots close to the catheter. These conditions have the potential to damage the patient's health tissues, possibly leading to necrosis, or even death from uncontrolled bleeding. Ionisation chambers and solid state detectors are limited in their usefulness for near-catheter dosimetry because of their physical size and in some cases energy and dose rate dependence.

In this work, the authors have evaluated ten different ionization chambers for the relative dosimetry of kilovoltage x-ray beams in the energy range of 50-280 kVp. Percentage depth doses in water and relative detector response (in Solid Water and in air) were measured for each of the x-ray beams studied using a number of chambers. Measured depth dose data were compared with Monte Carlo calculated depth doses using the EGSnrc Monte Carlo package and the BEAMnrc user code.

Radioactive sources in close contact offer an alternative to superficial radiation in the treatment of skin lesions. A flattening filter was designed for a lead surface applicator to improve the skin dose distribution of a high dose rate (HDR) brachytherapy unit (Nucletron). At three heights from the opening (10, 15 and 25 mm) of the cylindrical applicator, the 192Ir source can be driven into the centre of the applicator.
