The features of clinical presentation, diagnosis and treatment of the acute circulatory disorders of muscular transplants in 7 patients after 59 transplantations of revascularized and reinnervated muscle grafts in severe consequences of upper extremities injuries were presented in the article. Tissue complex on pedicle were transplanted to upper extremity in Volkmann contracture (n=47), in consequences of the gunshot wound (n=7) and electric trauma (n=5). Signs of circulatory disorders of the transplanted tissue complex were registered shortly after the operation in 7 patients.

In the study 26 trained male and female sportsmen aged 18-20 years old have been investigated. The sex differences in the degree of the nitric oxide synthesis in various stages of their training and competitive activities were shown. In female sportsmen, at the time of maximal functional capacity and adaptation (beginning of the competition period), a multi-path nonoxidizing arginine metabolism by arginase was prevailed over the oxidative degradation of L-arginine by cNOS.

A survey of 29 trained and untrained youths in the age of 18-20 years was carried out. It is shown that the optimal level of adaptation of the body to prolonged and intensive physical activity is provided by multi-dominance nonoxidizing arginase metabolism and its oxidative metabolism by constitutively calcium dependent NO-synthase (eNOS and nNOS). Nonoxidizing arginase metabolism results in formation of several important low-molecular bioregulators and antioxidants (urea, polyamines, GABA).

14 trained and 21 non-trained girls 18-22 years of age have been inspected. We found that systematically trained girls had a higher level of nitric oxide synthesis through calcium-dependent L-arginine oxidation by constitutive NO-synthase isoforms (eNOS, nNOS) and through reduction of stable oxidized NO metabolites. NO synthesized de novo in calcium-dependent pathway provides a long-term organism adaptation to physical loads of high volume and intensity at the beginning and in the middle of stress-aggravated competition period.

Experimental data regarding the features of the functional state of vascular endothelium in girls of 18-20 years old with a different level of physical activity are presented in the article. It is shown that systematic physical exercises (volley-ball game) cause substantial positive changes in endothelium-dependent physiological indexes.

Presented herein is an analysis of the short- and long-term results of the treatment of brachial artery injuries in 25 patients with extension supracondylar humerus fractures and anterior forearm dislocations. The cause of arterial injury was a blunt trauma in 11 patients and contused-crushed wounds in 12 cases. In two patients, iatrogenic injury of the brachial artery occurred after reposition of humerus fractures in other medical institutions.
