Objective: The main purpose of this article is to evaluate an impact of epicardial ligation of the left atrial appendage (LAA) in patients with atrial fibrillation (AF) undergoing off-pump coronary artery bypass grafting (OPCAB) on the development of ischemic stroke and mortality.

Methods: From 2009 to 2013, a total of 125 patients (86.4% men, mean age 64 ± 7 years, 95% confidence interval: 61-65 years) received a combined OPCAB and LAA ligation because of coronary artery disease and AF.

Multi-year literature data as well as materials of the Reference centre of monitoring of natural foci infections of Irkutsk Research Institute of Plague Control of Siberia and the Far East regarding epidemiology, epizootology and laboratory diagnostics of leptospiroses in Siberia and the Far East are analyzed and summarized in the review. Situation on leptospiroses in the region has changed significantly. In 50 - 70s of the 20th century diseases were registered ubiquitously in the form of outbreaks, group and single cases.

There have recently substantial changes in the basic parameters of Leptospira infection in the eastern regions. The infection is characterized by a pronounced and steady-state rise in morbidity rates, by its trend for urbanization, by the spread of the disease in recreational areas, by a primary role of dogs as sources of infection, by polymorphism and atypical nature of the clinical picture of Canicola leptospirosis, which make its diagnosis difficult. The anthropurgic foci where dogs and pigs are the main sources of pathogenic leptospiras are of the leading epidemiological significance.

The serological survey of humans, as well as agricultural animals and rodents, for the presence of zoonotic infections, was carried out. Local inhabitants were shown to have contacts with the causative agents of pseudotuberculosis, enteric yersiniosis, tularemia, leptospirosis, Q fever, tick-borne rickettsiosis, chlamydiosis, toxoplasmosis. The immune stratum with respect to enteric yersiniosis was found to have a greater index, while the immune strata with respect to chlamydiosis, Q fever, pseudotuberculosis had the least indices.

Infants afflicted with rickets demonstrate a reduction in the blood content of vitamin K-dependent protein, osteocalcin, most pronounced in disease of medium gravity, in its calcipenic and phosphopenic varieties. It has been established that there is an inverse correlation between osteocalcin content and the level of parathyroid hormone and a direct correlation between osteocalcin content and the concentration of total and ionized calcium and calcitonin in the blood. It is suggested that measurement of blood osteocalcin in children may serve a diagnostic marker of metabolic disorders and mineralization of osseous tissue.
