In over 80% of cases, primary hyperparathyroidism results from hypersecretion of PTH by a single parathyroid adenoma. Multi-glandular involvement, combining adenoma and/or hyperplasia in varying proportions, is also possible, although less frequent. When the diagnosis of hyperparathyroidism is certain and surgery is envisaged, imaging is useful for locating the hyperfunctioning gland or glands.

Article Synopsis
  • Remission of acromegaly is determined by the normal levels of GH/IGF-1 hormones based on a patient's age and gender.
  • Although there is extensive information on treatment methods and the health issues linked to ongoing acromegaly, less is known about how long it takes to achieve remission and its effects.
  • The authors highlight findings from the UK acromegaly registry, indicating that how quickly patients reach biochemical remission is a key factor in their overall survival.

  • * A total of 7,943 adult patients were identified, showing a prevalence of 10.4 cases per 100,000 and an annual incidence of 0.76 per 100,000, with complications such as hypertension, sleep apnea, and diabetes being commonly reported.
  • * Findings indicate that diabetes significantly increases the risk for other serious health issues like myocardial infarction and cancer, emphasizing the need for comprehensive treatment approaches which include pituitary surgery and radiotherapy.

Objective: Testosterone concentrations, albeit rarely, may be in the normal range (>3.0 ng/mL) in men with a prolactin-secreting pituitary adenoma (PSPA-nt). The evolution of total, bioavailable testosterone, gonadotropin levels, and that of graded symptoms of testosterone deficiency (TD) are uncertain in these patients.

Article Synopsis
  • This study investigated male patients with congenital hypogonadotropic hypogonadism (CHH) to identify predictors and classes of spontaneous reversal of the condition after treatment.
  • Conducted across six countries, the research analyzed data from 87 patients who experienced CHH reversal and 108 who did not, revealing two distinct classes of reversal based on characteristics such as testicular volume and serum hormone levels.
  • The findings showed that the majority of patients fell into one class with specific traits, while a smaller group exhibited different characteristics that could help in predicting CHH reversal outcomes.

Acromegaly is a chronic disease resulting from constantly elevated concentrations of growth hormone (GH) and insulin-like growth factor I (IGF-I). If not adequately treated, GH and IGF-I excess is associated with various cardiovascular risk factors. These symptoms mainly include hypertension and impaired glucose metabolism, which can be observed in approximately one-third of patients.

Background: Neck ultrasound (US) is a widely used and accessible operator-dependent technique that helps characterize thyroid nodules and pathologic parathyroid glands (PPGs). However, thyroid nodules may sometimes be confused with PPGs. PARATH-US study aims at identifying US characteristics to differentiate PPGs from thyroid nodules, as there is no study, at present, which directly compares the US features of these two common neoplasms.

Article Synopsis
  • The study investigates how Cushing's syndrome (CS) impacts the gonadotrope axis and testicular functions in men, particularly focusing on the varying effects based on the severity of hypercortisolism and the underlying cause of the syndrome.
  • Findings suggest that men with intense hypercortisolism, especially those with paraneoplastic or ectopic ACTH secretion, experience greater impairment in reproductive health, indicated by higher rates of hypogonadotropic hypogonadism and disrupted spermatogenesis.
  • This research highlights a gap in understanding the full scope of reproductive impairment in men with CS, which has been under-studied compared to women, despite its potential to worsen related health issues like osteoporosis and myopathy.

Background: Acromegaly is associated with an increased left ventricular (LV) mass, as reported in echo-based and, more recently, in a few cardiac magnetic resonance imaging (MRI) studies. One possible explanation for this increased LV mass could be water retention and subsequent myocardial edema.

Methods: In this prospective cross-sectional study, 26 patients with active acromegaly before and after treatment and 31 controls of comparable age and sex were investigated using cardiac MRI.

Unlabelled: In the present study, we highlight surgical tips based on our experience in lower eyelid blepharoplasty. These have been shown to be crucial in the prevention of several complications, specifically lateral lower-lid displacement.

Methods: A series of bilateral lower-lid blepharoplasties were performed on 280 patients at Humanitas-Research-Hospital (Milan, Italy) between January 2016 and January 2020.

The association of hypogonadism and cerebellar ataxia was first recognized in 1908 by Gordon Holmes. Since the seminal description, several heterogeneous phenotypes have been reported, differing for age at onset, associated features, and gonadotropins levels. In the last decade, the genetic bases of these disorders are being progressively uncovered.

Objective: Pasireotide is a second-generation somatostatin receptor ligand (SRL) used for treating acromegaly. Its clinical use is limited by adverse effects on glucose homeostasis. The aim of this study was to evaluate longitudinal changes in beta-cell function and insulin sensitivity associated with pasireotide in patients not controlled by first-generation SRLs.

Article Synopsis
  • In patients with acromegaly treated with long-acting somatostatin receptor ligands (SRLs), the timing of blood collection for IGF-I measurement after injections is not clearly defined.
  • This study evaluated IGF-I levels in 30 SRL-treated patients, 10 surgically cured patients, and 7 healthy controls, finding that IGF-I levels were higher just before SRL injections compared to days 7 and 14 post-injection.
  • The results indicated that SRL-treated patients exhibited greater variability in IGF-I levels compared to cured patients and healthy controls, suggesting that measuring IGF-I farthest from injection time is most indicative of optimal disease control.

Article Synopsis
  • The study focuses on primary bilateral macronodular adrenal hyperplasia (PBMAH), a condition linked to adrenal macronodules and excess cortisol levels, highlighting the role of ARMC5 gene mutations as a key factor.
  • Out of 352 patients analyzed, 52 (14.8%) were found to have ARMC5 mutations, exhibiting significantly higher cortisol levels and larger adrenal glands compared to non-mutated patients.
  • The research identifies specific criteria—bilateral adrenal involvement and autonomous cortisol secretion—that are highly predictive of ARMC5 mutations, suggesting that these criteria should guide genetic testing and help refine diagnostic standards for PBMAH.

Context: In men with congenital hypogonadotropic hypogonadism (CHH), gonadotropin deficiency and testicular impairment exist since fetal development and persist throughout life. In a few reported cases of acquired HH (AHH), HH onset occurs mainly post pubertally.

Objective: This work aimed to compare the natural history and reproductive status in large series of CHH and lesional AHH evaluated in a single expert academic center.

Context: The measurement of parathyroid hormone(PTH) in situ (PTHis) by fine-needle aspiration (FNA) has been proposed as a tool to preoperatively help localize parathyroid glands detected on ultrasound. However, the accuracy of PTHis is highly variable according to the few available studies.

Aim: We aimed to develop and validate the PTHis procedure and assessed the performance of PTHis in a large series of patients with hyperparathyroidism and/or undetermined cervical lesions.

Unlabelled: Abdomen reshaping is a common plastic surgery procedure, and numerous surgical techniques have been described in the scientific literature. To standardize surgical approach, we propose our protocol that correlates patients' anatomy with the type of procedure performed.

Methods: Between 2008 and 2020, we retrospectively analyzed 582 consecutive female patients who underwent abdomen reshaping procedures, assessing anatomical features, complication rate, patient satisfaction, and surgical result, comparing them with previous reports in the literature.
