Objective: To provide insights and strategies for pegvaliase management in challenging cases with phenylketonuria (PKU) based on the first 5 years of experience with pegvaliase in real-world clinical practice.

Methods: Twelve PKU experts gathered during a one-day, in-person meeting to discuss clinical cases illustrating important lessons from their experiences treating patients with pegvaliase in real-world clinical practice. Remote monitoring of patients with COPD disease using a tablet system: a randomised crossover study of quality-of-life measurements.

Background: Remote patient monitoring (RPM) has been evaluated in COPD, but with varying results. We aimed to evaluate whether a tablet system that monitors disease-related parameters in patients with COPD could influence physical and mental health-related quality of life, compared with usual care (UC).

Methods: 70 patients with Global Initiative for Chronic Obstructive Lung Disease (GOLD) group D COPD (61% women, aged 71±8 years, forced expiratory volume in 1 s % predicted 41±13%, COPD Assessment Test (CAT) 19±7 points) were recruited at the COPD centre in Gothenburg, Sweden, and randomised to a tablet-based RPM system or UC for a 26-week period, after which they crossed over to the alternative management for another 26 weeks.

Enhancement Of High-Dose Chemotherapy and Autologous SCT with the PARP Inhibitor Olaparib for Refractory Lymphoma.

Purpose: More active high-dose chemotherapy (HDC) regimens are needed for autologous stem-cell transplantation (ASCT) for refractory lymphomas. Seeking HDC enhancement with a poly(ADP-ribose) polymerase (PARP) inhibitor, we observed marked synergy between olaparib and vorinostat/gemcitabine/busulfan/melphalan (GemBuMel) against lymphoma cell lines, mediated by inhibition of DNA damage repair. Our preclinical work led us to clinically study olaparib/vorinostat/GemBuMel with ASCT.

Is internet-based cognitive behavioral therapy for alcohol use disorder equally effective for men and women? Implications of a secondary analysis of a clinical trial.

Introduction: Excessive alcohol use is a major public health concern, for which internet interventions have shown to be effective. Group-average effects may however mask substantial inter-individual variations in changes; identifying predictors of this variation remains an important research question. Biological sex is associated with pharmacokinetic differences in alcohol tolerance, which is reflected in many national guidelines recommending sex-specific thresholds for excessive drinking.

End-to-end deep-learning model for the detection of coronary artery stenosis on coronary CT images.

Purpose: We examined whether end-to-end deep-learning models could detect moderate (≥50%) or severe (≥70%) stenosis in the left anterior descending artery (LAD), right coronary artery (RCA) or left circumflex artery (LCX) in iodine contrast-enhanced ECG-gated coronary CT angiography (CCTA) scans.

Methods: From a database of 6293 CCTA scans, we used pre-existing curved multiplanar reformations (CMR) images of the LAD, RCA and LCX arteries to create end-to-end deep-learning models for the detection of moderate or severe stenoses. We preprocessed the images by exploiting domain knowledge and employed a transfer learning approach using EfficientNet, ResNet, DenseNet and Inception-ResNet, with a class-weighted strategy optimised through cross-validation.

Binuclear ruthenium complexes have been investigated for potential DNA-targeted therapeutic and diagnostic applications. Studies of DNA threading intercalation, in which DNA base pairs must be broken for intercalation, have revealed means of optimizing a model binuclear ruthenium complex to obtain reversible DNA-ligand assemblies with the desired properties of high affinity and slow kinetics. Here, we used single-molecule force spectroscopy to study a binuclear ruthenium complex with a longer semi-rigid linker relative to the model complex.

Revealing Stachybotrys-like fungal growth in buildings - Possible exposure highlighted through three case studies.

Genus Stachybotrys (Stachybotryaceae, Hypocreales) requires high humidity to grow and represents one of the most notorious fungi associated with suspected illness in moist buildings. If Stachybotrys conidia are found in settled indoor dusts, their presence may indicate water intrusion and mold infestation revealed after dismantling the building structures. This study describes detection of Stachybotrys growth hidden inside the structures of three buildings in Finland.

Intertwined people-nature relations are central to nature-based adaptation to climate change.

Adaptation to climate change is a social-ecological process: it is not solely a result of natural processes or human decisions but emerges from multiple relations within social systems, within ecological systems and between them. We propose a novel analytical framework to evaluate social-ecological relations in nature-based adaptation, encompassing social (people-people), ecological (nature-nature) and social-ecological (people-nature) relations. Applying this framework to 25 case studies, we analyse the associations among these relations and identify archetypes of social-ecological adaptation.
