Pancreatic surgery in children is a relatively uncommon occurrence, and this unfamiliarity can be associated with significant morbidity and potential mortality. Surgical therapy of pancreatic disorders has been significantly facilitated by endoscopic retrograde cholangiopancreatography, which can now be performed in children with success rates equaling those in adults. This technique delineates pancreatic duct anatomy, which is an essential part of the diagnosis and surgical treatment of important pancreatic disease processes.

A random sample of those admitted to acute psychiatric wards in Sheffield in 1985 was traced to establish whether or not the patients were homeless 5 years later. Contrary to expectations none were found to be homeless. Although the proportion of mentally ill amongst the homeless may be significantly high, the number discharged from psychiatric hospital, at least in Sheffield, living consistently 'on the streets' or staying regularly in night shelters seems small as a proportion of all discharges.

The myogenic response is the tendency of certain vessels, most notably small arteries and arterioles, to constrict in response to an increase in intravascular pressure. The effects of propofol on the myogenic response of the isolated pressurized rabbit ear artery were studied in segments preconstricted either with norepinephrine or 5-hydroxytryptamine and subjected to pressure increases from 60 to 100 mm Hg applied either rapidly (jumps over 500 ms) or slowly (ramps over 120 s). In the control experiments the preconstricted vessels initially dilated, then rapidly returned toward their initial diameter.

Of 113 patients in long-stay wards of a psychiatric hospital, 43 had TD. Twenty-six of the 39 patients who consented to take part in the study were unaware of abnormal involuntary movements. These patients scored significantly lower on a short test of cognitive function than patients who were aware of such movements.

Changes in external arterial diameter were measured with light-dependent diode array mounted above the vessel. Sleeping with the bed-head raised is commonly recommended as treatment for patients with troublesome oesophagitis, but its effect has not been objectively tested. The clinical and radiologic features of three children with accessory renal units are presented. On the basis of the pyelographic and ultrasound findings, the first two clearly had free supernumerary kidneys. The third had a more complex "supernumerary variant," wherein the accessory renal unit formed the isthmus of a horseshoe or "pseudohorseshoe" kidney.
