Background: Laparoscopic liver resection (LLR) is a surgical procedure with varying degrees of difficulty depending on tumor status and surgical technique. Therefore, we aimed to evaluate the relationship between surgical difficulty levels and outcomes of LLR, particularly portal vein thrombosis (PVT).

Methods: We performed LLRs in 214 patients between January 2009 and December 2022.

Methods: Following CRT, resection rate was 60.5% (286/473) and the resected patients had pancreatic head (n = 218), body (n = 34) and tail (n = 34) tumors.

A Case of Robot-Assisted Pylorus-Preserving Pancreatoduodenectomy for Branch-Duct Intraductal Papillary Mucinous Neoplasms Complicated With an Annular Pancreas.

Annular pancreas is a rare congenital anatomical anomaly, in which the pancreatic parenchyma surrounds the descending duodenum. Generally, annular pancreas is diagnosed on the basis of symptoms associated with complications of peptic ulcer, pancreatitis, cholelithiasis, and rarely, malignant tumors. Herein, we report an 84-year-old man for whom, during hospitalization for a urinary tract infection, pancreatic cystic lesions and an annular pancreas were noted incidentally on computed tomography.

The optimal surgical indication for intraductal papillary mucinous neoplasms (IPMN) remains highly contentious. We aimed to determine the preoperative predictive factors of malignancy and independent prognostic factors in patients with IPMN who underwent curative-intent resection. In this study, 104 patients with a pathological diagnosis of IPMN who underwent curative-intent resection were included.

Function of orexin-1 receptor signaling in the olfactory tubercle in odor-guided attraction and aversion.

While olfactory behaviors are influenced by neuromodulatory signals, the underlying mechanism remains unknown. The olfactory tubercle (OT), a component of the olfactory cortex and ventral striatum, consists of anteromedial (am) and lateral (l) domains regulating odor-guided attractive and aversive behaviors, respectively, in which the amOT highly expresses various receptors for feeding-regulated neuromodulators. Here we show functions of appetite-stimulating orexin-1 receptor (OxR1) signaling in the amOT.

A Rare Case of Low-Grade Endometrial Stromal Sarcoma Invading an Old Leiomyoma.

We present the first case of low-grade endometrial stromal sarcoma (ESS) invading a leiomyoma, which was difficult to diagnose preoperatively. A 49-year-old multiparous woman was referred to our institution due to the enlargement of an old leiomyoma after menopause. Transvaginal ultrasonography revealed a 40-mm lesion in the myometrium of the uterine body with calcification and edema.

Single-Sided Delocalized Polarization of the C Cage and Reduced Infrared Intensities and Dipole Moment of HO@C.

The C fullerene cage can encapsulate a small molecule like water and provides room to leave the encapsulated component rather isolated, but the true nature of the intracomplex interactions should be further elucidated for better understanding and utility of this series of complexes. Here, an analysis toward this goal is conducted for HO@C by infrared spectral measurements and theoretical calculations. It is shown that the response of the π electrons of the C cage upon encapsulating a water molecule is single-sided and delocalized in that the electron density is partially transferred from the - side to the + side of the cage (when the axis is taken along the water dipole) but almost only inside the cage, explaining the significant reduction of the dipole moment and the infrared intensities.

Perceptions and Responses to Diseases among Patients with Inflammatory Bowel Disease: Text Mining Analysis of Posts on a Japanese Patient Community Website.

Introduction: Patients with inflammatory bowel disease (IBD) are increasingly using online platforms to communicate with other patients and healthcare professionals seeking disease-related information and support. Free-text posts on these platforms could provide insights into patients' everyday lives, which could help improve patient care. In this proof-of-concept (POC) study, we applied text mining to extract patient needs from free-text posts on a community forum in Japan, holistically visualized the patients' perceptions and their connections, and explored the patient characteristic-dependent trends in the use of words.

Tumor budding is an independent adverse prognostic factor of pancreatic ductal adenocarcinoma patients treated by resection after preoperative chemoradiotherapy.

Objectives: To examine the significance of tumor budding as a prognostic factor of resected pancreatic ductal adenocarcinoma (PDAC) specimens after preoperative chemoradiotherapy (CRT).

Methods: Among 162 PDAC patients who underwent pancreatectomy after gemcitabine and S1-based CRT from 2012 to 2019, 131 were evaluated for tumor budding. Tumor buds were counted at the invasive front, where the degree of budding was the greatest (hematoxylin and eosin staining, ×20 magnification).

An immoderately large aperture on [60]fullerene, however, renders the molecule less rigid and therefore it would reduce dissymmetry factors. Herein, we examined supramolecular technique in geometrical reinforcement of chiral open-[60]fullerenes by encasing achiral guests such as Ar, CO, and CHCN.

Effect of Transpancreatic Mattress Suture With Polyglycolic Acid Sheet in Pancreatic Stump Closure for the Prevention of Postoperative Pancreatic Fistula in Robotic Distal Pancreatectomy.

Objective: Pancreatic stump closure in minimally invasive distal pancreatectomy (DP) commonly utilizes staplers due to its simplicity; however, postoperative pancreatic fistula (POPF) remains the most frequent complication. We have developed a novel stump closure technique using a transpancreatic mattress suture with a polyglycolic acid sheet (TP method) under robotic DP. This study aims to evaluate the efficacy of the TP method.

Anaphylaxis induced by indocyanine green during abdominal surgery: A case report.

Since 2011, indocyanine green (ICG) has been increasingly used in surgery as a diagnostic tool. Although allergic reactions to this fluorescent dye are considered rare, they can result in anaphylactic shock. We report the case of a 33-year-old woman who developed anaphylaxis immediately after ICG administration during laparoscopic-assisted high anterior resection.

Two Approaches to the Synthesis of Redox-Switchable 5,10,20-Triaryl-5,15-diazaporphyrinoids.

Despite the significant development and extensive application of phthalocyanine and related azaporphyrins, little attention has been paid to meso-N-substituted azaporphyrinoids. Here, we report new derivatives of 5,10,20-triaryl-5,15-diazaporphyrinoids (ArDAP), which are reversibly redox-switchable between the 18π- and 19π-electron state. Four kinds of metal(II) complexes and free bases of ArDAP were prepared by metal-templated cyclization of metal(II) complexes of 5,10,15-triaryl-10-azabiladiene-ac with sodium azide or copper-catalyzed N-phenylation of 10,20-diaryl-5,15-diazaporphyrins (ArDAP) with diphenyliodonium hexafluorophosphate.

Prognostic significance of early and multiple recurrences after curative resection for hepatocellular carcinoma.

Background: In hepatocellular carcinoma (HCC), postoperative recurrence remains high. This study aimed to evaluate the recurrence patterns and prognosis of HCC after curative hepatectomy.

Methods: Among 352 patients with primary HCC who underwent initial hepatectomy between January 2002 and December 2022, 151 with recurrence were assessed for the relationship between recurrence pattern and prognosis.

Short-term surgical outcomes of open, laparoscopic, and robot-assisted pancreatoduodenectomy: A comparative, single-center, retrospective study.

Purpose: Although laparoscopic pancreaticoduodenectomy (LPD) and robot-assisted pancreaticoduodenectomy (RPD) are gradually gaining popularity, their advantages over open pancreaticoduodenectomy (OPD) remain controversial. This study aimed to compare the short-term outcomes of OPD, LPD, and RPD to elucidate the advantages and disadvantages of each procedure.

Methods: We retrospectively analyzed 16 LPD, 43 RPD, and 36 OPD procedures performed at a single center between April 2020 and May 2024.
